Respuesta patológica a la quimioterapia neoadyuvante en mujeres menores de 35 años con cáncer de mama HER2 negativo

Abstract

Objetivo: Describir las tasas de respuesta patológica alcanzada por las pacientes menores de 35 años, con diagnóstico de cáncer de mama HER2 negativo, que recibieron quimioterapia neoadyuvante (QTNA). Métodos: Estudio observacional, descriptivo, retrospectivo de pacientes mujeres <35 años, con cáncer de mama HER2(-), EC: I-IIIC, atendidas en el INEN entre enero 2009 y diciembre 2014, que recibieron QTNA. Se realizó un análisis descriptivo de tasas de respuesta, variables clínico-patológicas, sobrevida libre de recurrencia (SLR) y sobrevida global (SG). Se comparó las variables clínico-patológicas, y sobrevida según el tipo de respuesta alcanzada. Resultados: 72 pacientes, media de edad de 32 años, estadio III (72.6%), que recibieron QTNA. El 80.6% recibió antraciclinas y taxanos secuenciales. El 13.7% alcanzó respuesta patológica completa (pCR). La SLR a los 60 y 120 meses se estimó en 57.7% y 48.8%. La SG a los 60 y 120 meses se estimó en 64.6% y 50.6%. La SLR fue estadísticamente superior en las pacientes que tuvieron pCR (p=0.049), y aunque la SG fue numéricamente superior, no alcanzó la significancia estadística (p=0.07). Conclusión: El presente estudio muestra tasas de respuesta patológica con quimioterapia neoadyuvante en mujeres muy jóvenes en una cohorte de mujeres latinas. Las tasas son comparables a estudios previos.

Aim: To describe the pathological response rates reached by patients younger than 35 years old, with human epidermal receptor 2 (HER2) negative breast cancer, after neoadjuvant chemotherapy. Methodology: This is an observational, cross sectional, retrospective study, of women younger than 35 years old, with HER2 negative breast cancer, Stage: I to IIIC, that received neoadjuvant chemotherapy at National Institute of Neoplastic Diseases (INEN) between January 2009 and December 2014. A descriptive analysis was performed in response rates, clinical and pathological variables, and survival times. Comparisons among clinical and pathological variables and survival according to the responses rates were performed. Results: Seventy-two patients, with an average age 32 years old, stage III (72.6%), received neoadjuvant chemotherapy. 80.6% received sequential anthracyclines and taxanes. 13.7% reached complete pathological response (pCR). Recurrence free survival at 60 y 120 months was estimated in 57.7% and 48.8%. Overall survival at 60 and 120 months was estimated in 64.6% and 50.6%. Recurrence free survival was statistically better in patients that reached pCR (p=0.049), however, overall survival was numerically but not statistically better (p=0.07). Conclusion: The present study shows the pathological response rates in a cohort of very young Latin-American women. The response rates are comparable to other studies.
