Evaluación tomográfica a largo plazo de la postura craneocervical post expansión rápida maxilar con anclaje óseo o dental

Abstract

Antecedentes: La expansión rápida maxilar (ERM) corrige la deficiencia transversal maxilar. Además, se observó que este tratamiento tiene efectos en las vías aéreas y la postura craneocervical, aunque la información es escasa y no hay estudios a largo plazo. Objetivo: Evaluar tomográficamente a largo plazo la postura craneocervical post ERM con anclaje óseo o dental. Materiales y Métodos: La muestra de este estudio retrospectivo fueron 162 tomografías clasificadas en 3 grupos: (1) Anclaje óseo, (2) Anclaje dental y (3) control. Se registraron las exploraciones con tomografía computarizada de haz cónico (TCHC) en 3 tiempos, T0 (antes del tratamiento), T1 (post-tratamiento) y T2 (2 años después del tratamiento). Se midió la postura craneocervical (PCC) con el software Xelis Dental Viewer, mediante los ángulos que están formados por la intersección de la horizontal de Frankfort y la línea odontoides (FH/OPT), el plano palatino y la línea odontoides (PP/OPT) y la línea mandibular y la línea odontoides (LM/OPT). La comparación entre grupos se llevó a cabo utilizando la prueba ANOVA. Resultados: En T1 no se encontraron cambios significativos en el ángulo craneocervical. Sin embargo, en T2 con anclaje óseo, los ángulos FH/OPT (p <0,048) y PP/OPT (p< 0,033) disminuyeron. Conclusiones: En el grupo con anclaje óseo, la angulación craneocervical disminuyó a largo plazo.

Background: Rapid maxillary expansion (RME) corrects maxillary transverse deficiency. In addition, this treatment was observed to have effects on airway and craniocervical posture, although information is scarce and there are no long-term studies. Objective: To evaluate long-term tomographic evaluation of post-RME craniocervical posture with bone or dental anchorage. Materials and Methods: The sample of this retrospective study was 162 tomographic scans classified into 3 groups (1) bone anchorage, (2) dental anchorage and (3) control. Cone beam computed tomography (CBCT) scans were recorded at 3 times, T0 (before treatment), T1 (post-treatment) and T2 (2 years after treatment). Craniocervical posture (CCP) was measured with Xelis Dental Viewer software, using the angles formed by the intersection of the Frankfort horizontal and the odontoid line (FH/OPT), the palatal plane and the odontoid line (PP/OPT) and the mandibular line and the odontoid line (ML/OPT). Comparison between groups was performed using ANOVA test. Results: At T1, no significant changes in craniocervical angle were found. However, at T2 with bone anchorage, the FH/OPT (p<0.048) and PP/OPT (p<0.033) angles decreased. Conclusions: In the bone anchored group, craniocervical angulation decreased in the long term.
