Características clínicas, bioquímicas y radiológicas en niñas con pubertad precoz central atendidas en un hospital público de Lima 2014-2019

Abstract

Antecedente:El diagnóstico de Pubertad precoz central(PPC) en niñas es frecuente, el ser tardío afecta la talla final adulta. Objetivo: Describir las características clínicas, bioquímicas y radiológicas en niñas con PPC atendidas en un hospital público de Lima entre el 2014-2019.Materiales y métodos: Serie de casos. Se revisó historias clínicas de niñas con diagnóstico de PPC.Se excluyó PPP, telarquia aislada y malformaciones.Se calculó el Z-IMC, Z-talla genética, Z-talla y PFTA. Se utilizó t-Student, t-test, Wilcoxon y Shapiro-Wilk, mediante el software STATA versión 17.Se consideró estadísticamente significativo p<0,05. El estudio fue aprobado por el CIE de la UPCH y del HCH.Resultados: Se revisaron 86 historias. La edad media cronológica fue 8,16±1,09 DE y la edad media ósea fue 10,33±1,27DE. Se encontró diferencia estadísticamente significativa (P=0,000) entre las medias de la edad ósea y la edad cronológica, el Z-talla de la niña y el Z-talla genética asimismo la PTFA y la TG. Se encontró M2(19,7%), M3(59,3%), M4(20,9%), normopeso(27,9%), sobrepeso (30,2%) y obesidad(41,8%). La mediana de LH fue 0,6(RIC:0,4-1,8) y FSH, 3,4(RIC:2,1-5,2).Conclusión: La edad cronológica al diagnóstico fue tardía con una edad ósea significativamente mayor respecto a la cronológica. La talla de la niña respecto a su TG fue significativamente mayor. La PTFA fue significativamente menor que la TG. La mayoría presentó desarrollo mamario avanzado, sobrepeso y obesidad. Es necesario capacitar y sensibilizar a los médicos para que la sospecha y referencia sea más temprana.

Background: The diagnosis of central precocious puberty (CPP) in girls is frequent, being late affects the final adult height. Objective: To describe the clinical, biochemical and radiological characteristics in girls with CPP attended in a public hospital in Lima between 2014-2019. Materials and methods: Case series. Clinical histories of girls with a diagnosis of CPP were reviewed.PPP, isolated thelarche and malformations were excluded.Z-IMC, genetic Z-height, Z-height and PFTA were calculated. T-Student, t-test, Wilcoxon and Shapiro-Wilk were used by STATA software 17 version. P<0,05 was considered statistically significant. The study was approved by the ICD of UPCH and HCH. Results: 86 histories were reviewed. Mean chronological age was 8,16±1,09SD and mean bone age was 10,33±1,27SD. Statistically significant differences (P=0,000) were found between the means of bone age and chronological age, the Z-height of the girl and the genetic Z-height as well as PTFA and TG. M2(19,7%), M3(59,3%), M4(20,9%), normal weight(27,9%), overweight (30,2%) and obesity(41,8%) were found. The median LH was 0,6(RIQ:0,4-1,8) and FSH, 3,4(RIQ:2,1-5,2) Conclusions: The chronological age at diagnosis was late with a significantly higher bone age than chronological. The height relative of girls regarding her TG was significantly higher. PTFA was significantly lower than TG. Most of them presented advanced breast development, overweight and obesity. It is necessary to train and sensitize physicians for earlier suspicion and referral.
