Cambios dentoalveolares del incisivo inferior evaluados por tomografía computarizada pre y postquirúrgica en pacientes con relación esquelética Clase III atendidos en el Centro Dental Docente de la Universidad Peruana Cayetano Heredia, 2025

Abstract

Introducción: En la corrección de la Clase III esquelética, la descompensación ortodóntica prequirúrgica proclina los incisivos inferiores dentro de los límites alveolares, cuyo rebase puede ocasionar dehiscencias o fenestraciones, mientras que el reposicionamiento quirúrgico posterior modifica adicionalmente la inclinación y la morfología de las tablas óseas vestibulares. La tomografía computarizada de haz cónico permite la evaluación sistemática de los cambios dentoalveolares perioperatorios en el incisivo inferior, lo que contribuye a la anticipación de riesgos periodontales y la optimización de la planificación terapéutica. Objetivo: Comparar características dentoalveolares del incisivo inferior mediante mediciones tomográficas entre el período prequirúrgico y el postquirúrgico en pacientes con relación esquelética Clase III atendidos en el Centro Dental Docente de la Universidad Peruana Cayetano Heredia durante el año 2025. Materiales y métodos: Se plantea un estudio observacional, retrospectivo y longitudinal, con inclusión de pares de tomografías computarizadas de haz cónico pre y postquirúrgicas de pacientes Clase III tratados con cirugía ortognática. Las mediciones de inclinación, altura y grosor vestibular se realizarán en el software Sidexis 4 tras un proceso de calibración inter e intra examinador que alcanzará un CCI≥0.75. El plan analítico incluirá estadística descriptiva, comparación bivariada de los cambios pre-post y regresión lineal múltiple para evaluar la asociación de dichos cambios con el sexo y el grupo etario, con significancia estadística fijada en p<0.05.Conclusión: El estudio cuantificará los cambios en inclinación, altura y grosor vestibular del incisivo inferior tras la cirugía ortognática, evidenciará su significación clínica y precisará la influencia del grupo etario y el sexo sobre dichas modificaciones, generando así evidencia aplicable al pronóstico periodontal y a la planificación terapéutica.

Introduction: In the correction of skeletal Class III malocclusion, pre-surgical orthodontic decompensation proclines the mandibular incisors within the alveolar limits; exceeding these boundaries may lead to dehiscences or fenestrations, while subsequent surgical repositioning further modifies the inclination and morphology of the vestibular bony plates. Cone-beam computed tomography enables the systematic evaluation of perioperative dentoalveolar changes in the mandibular incisor, thereby contributing to the anticipation of periodontal risks and the optimization of treatment planning. Objective: To compare dentoalveolar characteristics of the mandibular incisor through tomographic measurements between the pre-surgical and post-surgical periods in patients with skeletal Class III malocclusion treated at the Teaching Dental Center of the Universidad Peruana Cayetano Heredia during 2025. Materials and Methods: An observational, retrospective, longitudinal study is proposed, including pairs of pre- and post-surgical cone-beam computed tomography scans from Class III patients treated with orthognathic surgery. Measurements of inclination, height, and vestibular thickness will be performed using Sidexis 4 software following an inter- andintra-examiner calibration process that will achieve an ICC ≥ 0.75. The analytical plan will include descriptive statistics, bivariate comparison of pre-post changes, and multiple linear regression to evaluate the association of these changes with sex and age group, with statistical significance set at p < 0.05. Conclusion: The study will quantify the changes in inclination, height, and vestibular thickness of the mandibular incisor following orthognathic surgery, will demonstrate their clinical significance, and will specify the influence of age group and sex on such modifications, there by generating evidence applicable to periodontal prognosis and therapeutic planning.
