Sobrecarga del cuidador principal del paciente con enfermedad renal crónica

Abstract

La enfermedad renal crónica, al igual que otras enfermedades crónicas, es un problema de salud pública considerado un desafío global por su alta incidencia y prevalencia, así como también por su importante morbilidad y mortalidad. El deterioro progresivo de la función renal, genera dependencia en los pacientes, el mismo que requiere la participación del cuidador de estos mismos. Los cuidadores de dichos pacientes pueden sufrir sobrecarga con diferentes características, y puede ocasionar conflictos en diferentes aspectos de su vida. El presente trabajo tiene como titulo sobrecarga del cuidador principal del paciente con enfermedad renal crónica. Objetivo: describir la sobrecarga del cuidador principal. Metodología: es una revisión retrospectiva descriptiva de artículos de investigación. La muestra estuvo conformada de 198 artículos, se seleccionaron 32 que estuvieran alineados con el objetivo del presente trabajo. Los resultados más relevantes evidencian que existe sobrecarga en los cuidadores principales de aquellos pacientes, teniendo como evidencia que en 7 (21.90%) estudios se identificó una sobrecarga moderada, en 6 (18.8%) estudios se encontró una sobrecarga alta y en 4(12.5%) se identificó sobrecarga leve. Obteniendo como conclusión: La experiencia de cuidado siempre impacta en la vida de los cuidadores evidenciándose una sobrecarga alta, moderada y leve respectivamente. Más preponderante en cuidadores de pacientes con enfermedad renal crónica en terapia de restitución renal de hemodiálisis, seguida de pacientes que se encuentran en ambos tratamientos como es la diálisis peritoneal y hemodiálisis, y finalmente del tratamiento de diálisis peritoneal. Respecto al instrumento, se identificó la ejecución de la escala de Zarit, el más usado, viable y de fácil aplicación para la identificación de sobrecarga del cuidador principal. En cuanto a las dimensiones más afectadas, tenemos la psicológica seguida de la social, económica, física, salud y espiritual respectivamente, ocasionando un impacto negativo en la vida de los cuidadores principales de estos pacientes.

Chronic kidney disease, like other chronic diseases, is a public health problem considered a global challenge due to its high incidence and prevalence, as well as its significant morbidity and mortality. The progressive deterioration of kidney function generates dependency in patients, which leads to the involvement of their caregivers. Caregivers of patients with chronic kidney disease may experience overload with different characteristics, which can cause conflicts in different aspects of their lives. The present work is entitled "Overburden of the primary caregiver of patients with chronic kidney disease." The objective of this study was to describe the burden of primary caregivers. The methodology used was a descriptive retrospective review of research articles. The sample consisted of 198 articles, 32 of which were aligned with the objective of this work. The most relevant results showed that there is overload in the main caregivers of patients with chronic kidney disease, having as evidence that in 7 (21.90%) studies a moderate overload was identified, in 6 (18.8%) studies a high overload was found and in 4 (12.5%) mild overload was identified. The following conclusion was reached: The caregiving experience always impacts the lives of caregivers, demonstrating high,moderate, and mild burden, respectively. This burden is more prevalent in caregivers of patients with chronic kidney disease on hemodialysis renal restitution therapy, followed by patients on both treatments, such as peritoneal dialysis and hemodialysis, and finally peritoneal dialysis treatment, respectively. Regarding the instrument, the Zarit scale was identified as the most widely used, most viable, and easy to apply for identifying burden in the primary caregiver of a patient with chronic kidney disease. The most affected dimensions are the psychological dimension, followed by the social dimension, economic dimension, physical dimension, health dimension, and spiritual dimension, respectively, causing a negative impact on the lives of caregivers of patients with kidney disease.
