Estudio de caso en paciente mujer con problemas de consumo de alcohol que acude a CSMC San Alfonso de Paita 2024

Abstract

Se desarrolló un estudio de caso único, cuyo objetivo fue deshabituar el consumo compulsivo de alcohol en una paciente mujer de 33 años, mediante repertorios conductuales adaptativos en las áreas de personalidad, familiar y socioafectiva, atendida en el CSMC San Alfonso de Paita. Los hallazgos evaluativos precisaron personalidad dependiente con rasgos autodestructivos, patología severa con rasgos límite y síndrome de dependencia alcohólica. A nivel familiar, vínculos afectivos distantes con figuras parentales; a nivel socioafectivo, disfunción familiar severa, comunicación superficial, roles distorsionados y escasa capacidad adaptativa. Debido a su perfil, la paciente precisó abordaje cognitivo-conductual para la deshabituación del consumo. Se emplearon técnicas basadas en la teoría cognitivo conductual, psicoeducación, análisis funcional, reestructuración cognitiva, relajación muscular progresiva, autoinstrucciones, autocontrol, habilidades sociales, solución de problemas y prevención de recaídas Se concluyó que el abordaje cognitivo-conductual integral permitió desarrollar repertorios adaptativos que sustituyeron las conductas de consumo, logrando abstinencia sostenida en las 10 sesiones y seguimiento quincenal tras concluir las sesiones, confirmando la efectividad de las técnicas en la deshabituación alcohólica.

A single-case study design was conducted with the aim of achieving alcohol use cessation in a 33-year-old female patient through the development of adaptive behavioral repertoires across the personality, family, and socio-affective domains. The patient received treatment at the San Alfonso de Paita Community Mental Health Center. Assessment findings revealed a dependent personality pattern with self-defeating traits, severe personality pathology with borderline features, and an alcohol dependence syndrome. At the family level, emotionally distant bonds with parental figures were identified; at the socio-affective level, severe family dysfunction was evidenced, characterized by superficial communication, distorted role distribution, and limited adaptive capacity under stress. Given this clinical profile, the patient required a cognitive-behavioral intervention targeting alcohol use cessation. Treatment drew on evidence-based techniques grounded in cognitive-behavioral theory, including psychoeducation, functional analysis, cognitive restructuring, progressive muscle relaxation, self-instructional training, self-control strategies, social skills training, problem-solving, and relapse prevention. It was concluded that the comprehensive cognitive-behavioral approach enabled the development of adaptive behavioral repertoires that replaced alcohol use behaviors, achieving sustained abstinence across the 10 treatment sessions and biweekly follow-up assessments, thereby confirming the effectiveness of these techniques in treating alcohol dependence.
