Evaluación de deficiencia de hierro en donantes regulares de plaquetas por aféresis en un centro oncológico especializado durante el periodo 2023 al 2024

Abstract

Introducción: La donación frecuente de plaquetas por aféresis puede generar pérdidas mínimas pero progresivas de hierro representando un riesgo para su salud y continuidad para la donación. La hemoglobina reticulocitaria (Ret-he) es un marcador temprano para la detección de deficiencia de hierro presentando ventajas en comparación con ferritina. Objetivo: Evaluar la deficiencia de hierro en donantes regulares de plaquetas por aféresis utilizando como parámetro de estimación la medición de Ret-He en un centro oncológico especializado durante el periodo 2023 al 2024. Materiales y Métodos: Estudio observacional, analítico y transversal que se realizará en el Servicio de Banco de Sangre de un Centro Oncológico Especializado durante el año 2023 al 2024. Se incluirán 110 donantes de plaquetas por aféresis que cumplan los criterios de inclusión y exclusión. La Ret-He se cuantificará mediante el análisis en el equipo hematológico automatizado Sysmex XN 1000 y se recopilarán variables demográficas, clínicas y hematológicas. Se considerará la Ret-He con un punto de corte menor de 28 pg como deficiencia de hierro. Se realizarán pruebas estadísticas para evaluar la deficiencia de hierro con características demográficas, clínicas y hematológicas. Un p≤ 0.05 se considerará como estadísticamente significativo. Aspectos éticos: Es importante la protección de la base de datos de los participantes asegurando su confidencialidad. Este estudio será registrado en la plataforma del Sistema Descentralizado de Información y Seguimiento (SIDISI) de la Universidad Peruana Cayetano Heredia (UPCH). Se solicitará revisión y aprobación de consentimiento informado.

Introduction: Frequent platelet apheresis donation can result in minimal but progressive iron loss, posing a risk to the patient's health and continued donation. Reticulocyte hemoglobin (Ret-he) is an early marker for the detection of iron deficiency, offering advantages over ferritin. Objective: To evaluate iron deficiency in regular platelet donors by apheresis using the measurement of Ret-He as an estimation parameter in a specialized cancer center during the period 2023 to 2024. Materials and Methods: An observational, analytical, and cross-sectional study will be conducted at the Blood Bank Service of the National Institute of Neoplastic Diseases (INEN) during 2025. 110 platelet donors who meet the inclusion and exclusion criteria will be included by apheresis. Ret-He will be quantified by analysis on the Sysmex XN 1000 automated hematology equipment, and demographic, clinical, and hematological variables will be collected. Ret-He with a cut-off point of less than 28 pg will be considered iron deficiency. Statistical tests will be performed to assess iron deficiency with demographic, clinical, and hematological characteristics. A p≤ 0.05 will be considered statistically significant. Ethical Aspects: It is important to protect the participant database and ensure its confidentiality. This study will be registered on the Decentralized Information and Monitoring System (SIDISI) platform of the Universidad Peruana Cayetano Heredia (UPCH). Informed consent review and approval will be requested.
