Rehabilitación neuropsicológica computarizada en el deterioro cognitivo leve y la enfermedad de Alzheimer: revisión crítica de la literatura

Abstract

El deterioro cognitivo leve y la enfermedad de Alzheimer constituyeron dos de las principales condiciones neurodegenerativas asociadas al envejecimiento. En este contexto, la rehabilitación neuropsicológica computarizada cobró interés como alternativa o complemento a las intervenciones cognitivas tradicionales. Sin embargo, la evidencia disponible presentó resultados heterogéneos, lo que justificó una revisión crítica de la literatura. El objetivo fue analizar críticamente la evidencia científica sobre la rehabilitación neuropsicológica computarizada aplicada a adultos mayores con deterioro cognitivo leve y enfermedad de Alzheimer, identificando sus alcances, fortalezas, limitaciones, tendencias y vacíos de conocimiento. Se realizó una revisión crítica de la literatura científica publicada entre 2018 y 2025. El corpus final incluyó catorce investigaciones empíricas seleccionadas mediante criterios de inclusión y exclusión previamente establecidos. La información se analizó mediante síntesis temática y comparación crítica de los hallazgos, considerando dominios cognitivos, funcionalidad, adherencia terapéutica, realidad virtual, telerehabilitación y características metodológicas. Los estudios mostraron resultados heterogéneos. Se reportaron hallazgos favorables en memoria, atención y funciones ejecutivas, así como niveles generalmente adecuados de adherencia, aceptabilidad y usabilidad cuando estas variables fueron evaluadas. Los efectos sobre la funcionalidad y las actividades de la vida diaria fueron variables. Las diferencias observadas estuvieron asociadas al tipo de tecnología, la duración de las intervenciones y las características clínicas de las muestras. La evidencia revisada mostró un interés creciente por estas intervenciones; sin embargo, su heterogeneidad metodológica exigió interpretar los hallazgos con cautela y desarrollar investigaciones con diseños más robustos.

Mild Cognitive Impairment and Alzheimer's Disease were two of the main neurodegenerative conditions associated with aging. In this context, computerized neuropsychological rehabilitation gained increasing interest as an alternative or complementary approach to traditional cognitive interventions. However, the available evidence showed heterogeneous findings, which justified a critical review of the literature. The objective was to critically analyze the scientific evidence on computerized neuropsychological rehabilitation applied to older adults with Mild Cognitive Impairment and Alzheimer's Disease, identifying its scope, strengths, limitations, trends, and knowledge gaps. A critical review of the scientific literature published between 2018 and 2025 was conducted. The final corpus comprised fourteen empirical studies selected according to previously established inclusion and exclusion criteria. The information was analyzed through thematic synthesis and critical comparison of findings, considering cognitive domains, functionality, therapeutic adherence, virtual reality, telerehabilitation, and methodological characteristics. The studies showed heterogeneous findings. Favorable outcomes were reported in memory, attention, and executive functions, as well as generally adequate levels of adherence, acceptability, and usability when these variables were assessed. The effects on functionality and activities of daily living were variable. The observed differences were associated with the type of technology, the duration of the interventions, and the clinical characteristics of the study samples. The reviewed evidence showed growing interest in these interventions; however, its methodological heterogeneity required thatthe findings be interpreted with caution and highlighted the need for further research using more robust study designs.
