Resultados clínicos de aumentación y reconstrucción capsular superior con bíceps braquial en rotura masiva de manguito rotador

Abstract

Introducción: Las roturas masivas del manguito rotador representan un desafío quirúrgico por la alta tasa de falla en la reparación primaria. La reconstrucción capsular superior y la aumentación con tendón de la cabeza larga del bíceps surge como una opción biológica y costo-eficiente para disminuir la tasa de rerotura y fallo clínico. Objetivos: Evaluar los resultados clínicos del uso del tendón de la porción larga del bíceps en los desgarros masivos de manguito rotador. Métodos: Estudio retrospectivo y descriptivo en pacientes con roturas masivas de manguito rotador tratados mediante aumentación de bíceps y reconstrucción capsular superior en un centro de salud. Se registró dolor (EVA) y funcionalidad (Shoulder Subjective Value, SSV) en el preoperatorio, 3 meses y 6 meses postoperatorios. Resultados: Se evaluaron 14 pacientes de un mismo centro. Mediante t de Student para muestras pareadas se comparó el efecto de la intervención. El dolor preoperatorio promedio fue 8.21 ± 1.37, con mejoría significativa a 4.14 ± 1.77 a los 3 meses postoperatorio(p<0,0001) y a 2.57 ± 1.65 a los 6 meses postoperatorio (p<0,0001). El SSV aumentó de 32.86 ± 13.66 preoperatorio a 82.14 ± 10.73 (p<0,0001) a los 3 meses postoperatorios y a 83.57 ± 10.61 a los 6 meses postoperatorios. Un paciente presentó fallo clínico con rerotura evidenciada en resonancia magnética. Conclusiones: La aumentación con bíceps y reconstrucción capsular superior demostraron ser técnicas artroscópicas seguras y efectivas, con disminución significativa del dolor y mejoría funcional a corto plazo y mediano plazo.

Introduction: Massive rotator cuff tears represent a surgical challenge due to the high failure rate of primary repair. Superior capsular reconstruction and augmentation using the long head of the biceps tendon has emerged as a biological and cost-effective option to decrease the rupture rate and clinical failure. Objective: Evaluate clinical outcomes of using the long head of the biceps tendon in massive rotator cuff tears. Methods: Retrospective study of patients with massive rotator cuff tears treated with biceps tendon augmentation and superior capsule reconstruction at one health care center. Pain (VAS) and function (Shoulder Subjective Value, SSV) were assessed preoperatively, at 3 months, and at 6 months postoperatively. Results: 14 patients of one center were assessed. Statistical analysis using paired student's t-tests was used to compare the effect of theintervention. The mean preoperative pain score was 8.21 ± 1.37, with significantimprovement to 4.14 ± 1.77 at 3 months postoperatively (p < 0.0001) and to 2.57 ±1.65 at 6 months postoperatively (p < 0.001). The SSV score increased from 32.86± 13.66 preoperatively to 82.14 ± 10.73 (p < 0.0001) at 3 months postoperatively and to 83.57 ± 10.61 at 6 months postoperatively. One patient experienced clinical failure with MRI showing re rupture. Conclusions: Biceps augmentation and Superior Capsule Reconstruction proved to be safe and effective arthroscopic techniques, with significant pain reduction and functional improvement in the shortand medium term.
