Evaluación tomográfica de la cavidad nasal post expansión maxilar con anclaje esquelético en pacientes del Servicio de Ortodoncia de la Clínica Peruana Cayetano Heredia entre los años 2008 - 2023

Abstract

Introducción: La expansión rápida del maxilar asistida con mini implantes (MARPE) es un tratamiento empleado para solucionar la deficiencia transversal maxilar en pacientes adultos, ofrece ventajas en la preservación ósea y mejora de la respiración. La cavidad nasal, vital para la respiración y el desarrollo facial, puede verse afectada por la hipoplasia maxilar, lo que resalta la importancia de analizar las modificaciones en las vías aéreas tras el tratamiento con MARPE mediante tomografía computarizada de haz cónico (TCHC). Objetivo: Evaluar los cambios a nivel de la cavidad nasal en pacientes que han sido sometidos a tratamiento de expansión maxilar con anclaje esquelético en la Clínica Estomatología Cayetano Heredia entre los años 2008 -2023 mediante TCHC. Materiales y métodos: En el presente estudio retrospectivo, se revisarán tomografías de pacientes que recibieron tratamiento de MARPE, que acudieron al servicio de Ortodoncia y Ortopedia maxilar del Centro Dental Docente Cayetano Heredia, que cumplan con los criterios de inclusión y exclusión. Se deberá contar con TCHC de pacientes con MARPE pretratamiento y postratamiento. Se realizarán las mediciones en el software Dolphin imaging para determinar las variables de volumen y área de la cavidad nasal. El análisis bivariado se realizará utilizando la prueba Chi cuadrado, con un nivel de confianza del 95% y p<0.05. Conclusiones: Se determina que los resultados esperados permitirán identificar los cambios en el área y volumen de la cavidad nasal en pacientes sometidos a MARPE, además de evaluar si hay diferencias significativas entre hombres y mujeres.

Introduction: Rapid maxillary expansion assisted with mini-implants (MARPE) is a treatment used to address maxillary transverse deficiency in adult patients, offering advantages in bone preservation and respiratory improvement. The nasal cavity, vital for breathing and facial development, can be affected by maxillary hypoplasia, highlighting the importance of analyzing airway modifications after MARPE treatment using cone-beam computed tomography (CBCT). Objective: To evaluate changes in the nasal cavity in patients who underwent maxillary expansion with skeletal anchorage at the Cayetano Heredia Stomatology Clinic between 2008 and 2023 using CBCT. Materials and Methods: In this retrospective study, tomographies of patients treated with MARPE who visited the Orthodontics and Maxillary Orthopedics service at the Cayetano Heredia Dental Teaching Center will be reviewed, adhering to inclusion and exclusion criteria. Pre-treatment and post-treatment CBCT scans of MARPE patients will be required. Measurements will be conducted using Dolphin imaging software to determine the volume and area variables of the nasal cavity. Bivariate analysis will be performed using the Chi-square test, with a confidence level of 95% and p<0.05. Conclusions: It is determined that the expected results will identify changes in the area and volume of the nasal cavity in patients undergoing MARPE, as well as assess if there are significant differences between men and women.
