Aloinmunización por anticuerpos anti D, C, c, E y e en mujeres en edad fértil

Abstract

ANTECEDENTES: La aloinmunización por anticuerpos contra antígenos del sistema Rh es una respuesta inmunológica que puede desarrollarse en pacientes transfundidos, con procesos autoinmunes o mujeres embarazadas. Su relevancia clínica es crítica en mujeres en edad fértil, ya que, al recibir transfusiones antes del embarazo, pueden desarrollar anticuerpos que afecten al feto en gestaciones posteriores, como la enfermedad hemolítica del recién nacido, incompatibilidad sanguínea y hasta la muerte perinatal. OBJETIVO: Describir la aloinmunización por anticuerpos anti D, C, c, E y e en mujeres en edad fértil. TIPO DE ESTUDIO: Se realizó una revisión bibliográfica de investigaciones científicas publicadas entre el 2019 y 2024. CONCLUSIÓN: La aloinmunización en mujeres en edad fértil representa un riesgo significativo para la salud materno fetal y más aún si recibieron transfusiones previas, el principal factor asociado fue la politransfusión con una prevalencia del 62.5 %, seguido del embarazo 25.0%, inmunoprofilaxis 6.2% y leche materna 6.2%. Se identificaron los anticuerpos Rh anti-D como los más frecuentes, seguido de anti-E, anti-C y anti-c. Estos hallazgos refuerzan la necesidad de protocolos preventivos, como estudios inmunohematológicos y la aplicación oportuna de inmunoglobulina anti-D.

BACKGROUND: Alloimmunization due to antibodies against antigens of the Rh blood group system is an immune response that may develop in transfused patients, individuals with autoimmune conditions, or pregnant women. Its clinical relevance is critical in women of childbearing age, since transfusions received prior to pregnancy may lead to the development of antibodies that can affect the fetus in subsequent pregnancies, resulting in hemolytic disease of the newborn, blood group incompatibility, and even perinatal death. OBJECTIVE: To describe alloimmunization caused by anti-D, anti-C, anti-c, anti-E, and anti-e antibodies in women of childbearing age. STUDY DESIGN: A bibliographic review of scientific studies published between 2019 and 2024 was conducted. CONCLUSION: Alloimmunization in women of childbearing age represents a significant risk to maternal–fetal health, particularly in those with a history of prior transfusions. The main associated factor was multiple transfusions, with a prevalence of 62.5%, followed by pregnancy (25.0%), immunoprophylaxis (6.2%), and breast milk exposure (6.2%). Anti-D Rh antibodies were identified as the most frequent, followed by anti-E, anti-C, and anti-c. These findings reinforce the need for preventive protocols, including immunohematological testing and the timely administration of anti-D immunoglobulin.
