Publicación: Polimorfismos de los genes enzimáticos NAT2, CYP2E1 y GST y presencia de reacción adversa a fármacos antituberculosos en la población peruana

En el Perú, 24 581 personas fueron diagnosticadas con Tuberculosis (TB) en el 2020. Si bien el tratamiento de la TB es efectivo, el 3.4-13% se asocia con reacciones adversas a medicamentos (RAM) significativas, considerándose la lesión hepática inducida por medicamentos (DILI por sus siglas en inglés) como la más predominante en el Perú. Entre los medicamentos antituberculosos de primera línea, la isoniazida (INH) es la principal causa de la aparición de DILI. En el hígado, la INH se metaboliza por las enzimas N-acetiltransferasa-2 (NAT2) y el citocromo P450 2E1 (CYP2E1) y Glutation S transferasa (GST) con dos isoformas GSTT1 y GSTM1. Poco se conoce sobre si las características genéticas del paciente infectado por TB en el Perú podrían estar asociadas a la presencia de reacciones adversas a fármacos. En el presente estudio exploramos con dos preguntas de investigación si el fenotipo acetilador NAT2, genotipo CYP2E1 y el genotipo GST con sus dos isoformas estarían asociados a la presencia de reacciones adversas en pacientes peruanos infectados por tuberculosis. Investigación 1: Estudio transversal que evaluó las interacciones entre el genotipo lento CYP2E1 y los acetiladores lentos NAT2 y su asociación con la presencia de DILI en pacientes con TB. Se evaluó 377 participantes y encontramos que el acetilador intermedio NAT2 es el menos prevalente entre los pacientes con reacciones adversas (p=0.24). Además, reportamos que la combinación de acetiladores intermedios NAT2 y el genotipo CYP2E1 c1/c1 presenta una asociación débil de protección (OR=0.13; p=0.050) contra el desarrollo de DILI en nuestra población. Investigación 2: Estudio transversal que evaluó las interacciones entre los genotipos GSTT1 y GSTM1 para asociarse a la presencia de DILI en pacientes con TB. Se evaluó 377 participantes y encontramos que ningún genotipo fue prevalente en los pacientes que desarrollaron DILI. Además, reportamos que la combinación del genotipo presente GSTM1, genotipo nulo GSTT1, acetiladores rápidos de NAT2 y genotipo CYP2E1 c1/c1 tenían un riesgo significativo para el desarrollo de DILI (OR = 11; p = 0.017) en nuestra población. Por lo tanto, proponemos que la presencia del acetilador intermedio NAT2 y el genotipo c1/c1 de CYP2E1 podría ayudar en la monitorización terapéutica de fármacos al ser un posible factor protector de la presencia de DILI. Además, que la presencia del genotipo silvestre GSTM1, genotipo nulo GSTT1, acetiladores rápidos de NAT2 y genotipo CYP2E1 c1/c1 en la población de estudio podrían considerarse como un factor de riesgo para el desarrollo de RAM debido a la ingesta terapéutica de fármacos.
Resumen
In Peru, 24,581 people were diagnosed with tuberculosis (TB) in 2020. Although TB treatment is effective, 3.4-13% is associated with adverse drug reactions (ADR), considering the Drug-Induced Liver Injury (DILI) as the most predominant in Peru. Among the first-line antituberculous medications, Isoniazide (INH) is the main cause of DILI's appearance. In the liver, INH is metabolized by the enzymes N- N-acetyltransferase -2 (NAT2) and the cytochrome P450 2E1 (CYP2E1) and Glutation S Transferase (GST) with two GSTT1 and GSTM1 isoforms. Little is known about whether the genetic characteristics of the patient infected with TB in Peru could be associated with the presence of adverse drug reactions. In the present study we explore with two research questions if the NAT2 acetylator phenotype, CYP2E1 genotype and the GST genotype with their two isoforms would be associated with the presence of adverse reactions in Peruvian patients infected with tuberculosis. Research 1: Transversal study that evaluated the interactions between the slow CYP2E1 genotype and the slow NAT2 acetylator and their association with the presence of DILI in patients with TB. 377 participants were evaluated and we found that the NAT2 intermediate acetylator is the least prevalent among patients with adverse reactions (p = 0.24). In addition, we report that the combination of NAT2 intermediate acetylator and the CYP2E1 C1/C1 genotype presents a weak protection association (OR = 0.13; p = 0.050) against the development of DILI in our population. Research 2: Transversal study that evaluated the interactions between GSTT1 and GSTM1 genotypes to associate with the presence of DILI in patients with TB. 377 participants were evaluated and we found that no genotype was prevailing in patients developed DILI. In addition, we report that the combination of the GSTM1 present genotype, null GSTT1 genotype, rapid NAT2 acetylator and CYP2E1 C1/C1 genotype had a significant risk for the development of DILI (OR = 11; p = 0.017) in our population. Therefore, we propose that the presence of NAT2 intermediate acetylator and C1/C1 genotype of CYP2E1 could help in drug therapeutic monitoring as it is a possible protective factor of the presence of DILI. In addition, that the presence of the GSTM1 wild genotype, null GSTT1 genotype, rapid NAT2 acetylator and CYP2E1 C1/C1 genotype in the study population could be considered as a risk factor for the development of RAM due to drug therapeutic intake.
