Evaluación tomográfica de las dimensiones de la placa ósea mandibular en pacientes con relación esquelética clase III atendidos en el Centro Dental Docente Cayetano Heredia entre los años 2008 - 2023

Abstract

Introducción: La maloclusión esquelética clase III puede ser causada por prognatismo mandibular, retrognatismo maxilar o ambas. Los tratamientos de compensación incluyen la distalización de toda la arcada inferior con minitornillos extra-radiculares, siendo la placa ósea mandibular el sitio de elección para su inserción. Existe evidencia de las variaciones anatómicas en la región; por lo tanto, es necesario asociar estos hallazgos respecto a la edad y sexo del paciente. Objetivo: Evaluar las dimensiones de la placa ósea mandibular (POM) en tomografías de pacientes con relación esquelética clase III atendidos en el Centro Dental Docente de la Universidad Cayetano Heredia entre los años 2008-2023. Materiales y métodos: Se seleccionarán a los pacientes que cumplan con los criterios de inclusión y exclusión a partir de historias clínicas y tomografías del Servicio de Ortodoncia y Ortopedia Maxilar. Las tomografías serán evaluadas en el programa Real Scan 2.0 para la ubicación de puntos cefalométricos, trazados y mediciones de las dimensiones de la POM. La altura y grosor cortical se medirán según lo propuesto por Chang et al. Finalmente, para todo el análisis, se procesarán los datos en el programa estadístico STS. Conclusiones: No se esperan diferencias en las dimensiones de la POM respecto al rango de edad, sin embargo, hay evidencia de variaciones según el sexo del paciente. Se sugiere realizar futuros estudios que incluyan mayores rangos de edad y presencia de estructuras aledañas.

Introduction: Skeletal class III malocclusion occurs due to mandibular prognathism, maxillary retrognathism, or a combination of both. Compensatory treatments include distalization of the entire lower arch with extra-radicular miniscrews, with the mandibular bone plate being the site of choice for their insertion. There is evidence of anatomical variations in the region, therefore it is necessary to associate these findings with respect to the sex and age of the patient. Objective: To evaluate the dimensions of the mandibular buccal shelf (MBS) in computed tomography (CT) scans of patients with class III skeletal relationship treated at the Cayetano Heredia University Dental Teaching Center between 2008- 2023. Materials and Methods: Information will be collected from the clinical histories and CT scans of patients from the Orthodontics and Maxillary Orthopedics service, selecting those that meet the inclusion and exclusion criteria. The CT scans will be evaluated in the Real Scan 2.0 program for the location of cephalometric points, tracings and measurements of the POM dimensions. The cortical height and thickness will be measured as proposed by Chang et al. Finally, for the entire analysis, the data will be processed in the STS statistical program. Conclusions: No differences in the POM dimensions are expected with respect to the age range, however, there is evidence of variations according to the sex of the patient. It is suggested to carry out future studies that include wider age ranges and the presence of surrounding structures.
