Percepción estética de la sonrisa según región geográfica en cirujanos dentistas y población general del Perú: un análisis comparativo

dc.description.abstractIntroducción: La percepción estética de la sonrisa es un constructo multidimensional influenciado por factores culturales, geográficos y por la formación odontológica del observador. En el Perú, la diversidad geográfica y étnica constituye una variable clínicamente relevante aún poco documentada en la literatura nacional. Objetivo: Comparar la percepción estética de la sonrisa según región geográfica entre cirujanos dentistas (CD) y personas sin formación odontológica(PSF) del Perú. Materiales y métodos: Estudio observacional, transversal y comparativo en 794 participantes de tres regiones: costa (n=267), sierra (n=277) y selva (n=250). Se utilizaron tres diseños de sonrisa generados con inteligencia artificial: imagen base estandarizada (A) obtenida con Artlist®; y dos diseños generados con SmileCloud®, morfología triangular y menor simetría (B) y morfología cuadrangular y mayor simetría (C). La importancia atribuida a características estéticas intraorales y faciales se midió con escala Likert de cinco puntos; la preferencia por la sonrisa más atractiva mediante selección excluyente. Resultados: Se identificaron diferencias significativas en la importancia atribuida a parámetros estéticos entre CD y PSF en las tres regiones (p < 0.05). La selva registró la mayor brecha, con un patrón inverso donde las PSF otorgaron valoraciones más elevadas que los CD. La asociación entre grupo evaluador e impacto de la apariencia en la autoestima fue significativa en las tres regiones (𝑝<0.05). A través de un modelo de regresión logística multinomial globalmente significativo (𝑝<0.001), se identificó que la formación profesional fue el principal predictor independiente, con los CD mostrando mayor tendencia a elegir morfología triangular sobre cuadrangular (𝑂𝑅=1.497; IC95%: 1.262–1.774; 𝑝<0.001). Conclusión: La importancia atribuida a los componentes estéticos y la preferencia por diseños dentales varían según región geográfica y grupo evaluador, evidenciando la necesidad de incorporar la diversidad sociocultural regional como variable clínicamente relevante en la planificación estética odontológica.spa
dc.description.abstractIntroduction: The aesthetic perception of the smile is a multidimensional construct influenced by cultural and geographic factors, as well as the dental training of the observer. In Peru, geographic and ethnic diversity constitutes a clinically relevant variable that remains poorly documented in the national literature. Objective: To compare the aesthetic perception of the smile according to geographic region between dental surgeons (DS) and individuals without dental training (IWDT) in Peru. Materials and methods: An observational, cross-sectional, and comparative study conducted in 794 participants from three regions: coast (n=267), sierra (n=277), and amazon (n=250). Three AI-generated smile designs were used: a standardized base image (A) obtained with Artlist®, and two designs generated with SmileCloud® — triangular morphology with lower symmetry (B) and quadrangular morphology with higher symmetry (C). The importance attributed to intraoral and facial aesthetic characteristics was measured using a five-point Likert scale; preference for the most attractive smile was assessed through exclusive selection. Results: Significant differences were identified in the importance attributed to specific aesthetic parameters between DS and IWDT across all three regions (p < 0.05). The amazon region recorded the greatest perceptual gap, with an inverse pattern in which IWDT assigned higher ratings than DS. The association between evaluator group and the perceived impact of appearance on self-esteem was significant in all three regions (p < 0.05). Through a globally significant multinomial logistic regression model (p < 0.001), professional training was identified as the main independent predictor, with DS showing a greatertendency to choose triangular over quadrangular morphology (OR = 1.497; 95%CI: 1.262–1.774; p < 0.001). Conclusion: The importance attributed to aesthetic smile components and the preference for dental designs vary according to geographic region and evaluator group, highlighting the need to incorporate regional sociocultural diversity as a clinically relevant variable in aesthetic dental treatment planning.eng
