| spa |
| dc.description.abstract | Introduction: Advanced therapy medicinal products (ATMPs) —gene therapy, somatic/adoptive cell therapy, and tissue engineering— pose particular challenges for pharmacovigilance due to their biological complexity, potential for delayed effects, and need for prolonged follow-up. In South America, regulation is heterogeneous, with harmonization gaps that make it relevant to map requirements, capacities, and opportunities for regional convergence. Objective: To map the regulatory frameworks and pharmacovigilance requirements applicable to ATMPs in South American countries, identifying similarities, differences, and gaps compared to international standards (ICH E2E, GVP/RMP, LTFU guidelines), including the role of RWE in post-marketing surveillance.Materials and Methods: Scoping review following JBI methodology and reported according to PRISMA-ScR. Sources included PubMed/MEDLINE, Embase, LILACS, SciELO, complementary sources (Google Scholar, Semantic Scholar, Elicit), regulatory agency websites, PAHO/PANDRH, and the ALICIA Peru repository, prioritizing publications from 2015–July 2026. Data extraction used an adapted JBI instrument capturing variables such as RMP, PSUR/PBRER, LTFU, traceability, PASS/PAES, SAE reporting timelines, and RWD/RWE use, synthesized through narrative and comparative analysis. Results: Forty-one documents were included. Only Brazil, Argentina, and Chile have ATMP-specific regulation; the remaining countries apply general provisions. Individual ADR reporting was universal (28/41). Proactive components were scarce: RMP (5 countries), PSUR (4), PASS/PAES (3), traceability (3). LTFU was formalized only in Brazil, Argentina, and French Guiana (up to 15 years). SAE reporting timelines ranged from <24 hours (Peru) to 15 days. RWD/RWE was documented in 18/41 records, led by Brazil. Conclusion: The region shows asymmetric progress; critical gaps persist in LTFU, post-authorization studies, and harmonization, requiring a collaborative regional strategy with harmonized minimum requirements, multinational registries, and safety signal exchange. | eng |
