“Relación entre las características clínicas y biomarcadores virológicos e inmunológicos, en pacientes con paraparesia espástica tropical (TSP) asociada a la infección por virus linfotrópico T humano tipo 1 (HTLV-1)”

Abstract

La paraparesia espástica tropical (TSP) es un trastorno neurológico crónico que está relacionado con la infección por el virus linfotrópico de células T humano tipo 1 (HTLV-1). Se caracteriza por la aparición gradual de debilidad muscular y espasticidad en extremidades inferiores, así como afectación de esfínteres de forma progresiva. La TSP afecta predominantemente a personas de regiones endémicas, procedentes del Caribe, África y Sudamérica. Los biomarcadores, como la neopterina, CXCL10 y quimiocinas inflamatorias, han demostrado ser valiosos para comprender la progresión de la enfermedad y su asociación con la inflamación del sistema nervioso central. Estos biomarcadores también pueden ser importantes para predecir la progresión de la TSP y dirigir las intervenciones terapéuticas. Objetivo general: Realizar una revisión bibliográfica para establecer las características clínicas y biomarcadores virológicos e inmunológicos de la infección por HTLV1 en pacientes con TSP que nos permita dilucidar la problemática que representa la ausencia de información fiable para la toma de decisiones en cuanto a diagnóstico, pronóstico, tratamiento y seguimiento de dicha patología. Metodología: Procedimiento escalonado consistente en: búsqueda, selección, revisión, interpretación y síntesis de la evidencia científica. Como bases de datos se utilizó: PubMed, Cochrane, Scielo, LILACS, Science Direct, Dialnet y Google Académico bajo los descriptores MeSH y términos clave: Paraparesis, Tropical Spastic, HTLV-1 Infections, Human T-lymphotropic virus 1, Biomarkers. Se aplicaron criterios de inclusión y exclusión para determinar la elegibilidad de la bibliografía, lo que llevó al análisis de 19 artículos que cumplían con los criterios de selección. Conclusiones: Sobre la base de los hallazgos de este estudio, se busca concluir que la investigación científica centrada en los aspectos inmunológicos, clínicos y virológicos del HTLV-1 y el desarrollo de la TSP es de suma importancia, particularmente en los países que carecen de datos sobre esta relación patológica.

Tropical spastic paraparesis (TSP) is a chronic neurological disorder that is related to infection with human T-cell lymphotropic virus type 1 (HTLV-1). It is characterized by the gradual onset of muscle weakness and spasticity in the lower extremities, as well as progressive sphincter involvement. TSP predominantly affects people from endemic regions, originating from the Caribbean, Africa and South America. Biomarkers, such as neopterin, CXCL10 and inflammatory chemokines, have proven valuable in understanding disease progression and its association with central nervous system inflammation. These biomarkers may also be important in predicting the progression of PSD and directing therapeutic interventions. Overall objective: To conduct a literature review to establish the clinical characteristics and virological and immunological biomarkers of HTLV1 infection in patients with TSP, in order to elucidate the problem represented by the lack of reliable information for decision making regarding diagnosis, prognosis, treatment, and follow-up of this pathology. Methodology: Stepwise procedure consisting of: search, selection, review, interpretation and synthesis of scientific evidence. The following databases were used: PubMed, Cochrane, Scielo, LILACS, Science Direct, Dialnet and Google Scholar under the MeSH descriptors and key terms: Paraparesis, Tropical Spastic, HTLV-1 Infections, Human T-lymphotropic virus 1, Biomarkers. Inclusion and exclusion criteria were applied to determine the eligibility of the literature, leading to the analysis of 19 articles that met the selection criteria. Conclusions: Based on the findings of this study, we seek to conclude that scientific research focused on the immunological, clinical and virological aspects of HTLV-1 and the development of PHT is of utmost importance, particularly in countries lacking data on this pathological relationship.
