Evaluación tomográfica de la altura del hueso palatino en pacientes con deficiencia transversal del maxilar del Servicio de Ortodoncia de la Clínica Docente UPCH 2009 a 2023

Abstract

Introducción: La deficiencia transversal del maxilar es un dilema que se da con frecuencia en pacientes adultos. La expansión rápida convencional produce mayores efectos dentoalveolares y pocos efectos a nivel esqueletal, es por ello que el uso de los mini-implantes amplía las posibilidades de tratamiento. Ultimamente se han propuesto diferentes dispositivos de expansión soportados en mini-implantes para producir una apertura de la sutura media palatina y corregir el problema transversal. Actualmente se describe que el anclaje esquelético bicortical ofrece una mejor estabilidad y distribución de fuerzas, por lo tanto, es muy importante evaluar la altura del hueso palatino en pacientes con deficiencia transversal del maxilar para determinar las zonas de inserción ideales de los mini-implantes. Objetivo: Evaluar la altura del hueso palatino en pacientes con deficiencia transversal del maxilar mediante tomografía computarizada de haz cónico (TCHC). Materiales y métodos: en este estudio descriptivo, se realizará el registro de las dimensiones de la altura en las fichas de recolección de datos para posteriormente analizar los resultados de acuerdo a las variables del estudio y construir tablas en función a la información obtenida. Conclusiones: este estudio nos ayudará a determinar la mejor ubicación y tamaño de los miniimplantes para el anclaje bicortical. Esto resultará en una mayor estabilidad, menos deformación y fractura de los miniimplantes, mayor expansión paralela en el plano coronal y mayor expansión durante la expansión transmitida en el hueso.

Introduction: Transverse deficiency of the maxilla is a dilemma that frequently occurs in adult patients. Conventional rapid expansion produces greater dentoalveolar effects and few effects at the skeletal level, which is why the use of mini-implants expands the treatment possibilities. Lately, different expansion devices supported on mini-implants have been proposed to produce an opening of the midpalatal suture and correct the transverse problem. It is currently described that bicortical skeletal anchorage offers better stability and distribution of forces, therefore, it is very important to evaluate the height of the palatal bone in patients with transverse maxillary deficiency to determine the ideal insertion areas of the mini-implants. Objective: To evaluate the height of the palatine bone in patients with transverse maxillary deficiency using cone beam computed tomography (HCCT). Materials and methods: in this descriptive study, the height dimensions will be recorded in the data collection sheets to later analyze the results according to the study variables and build tables based on the information obtained. Conclusions: this study will help us determine the best location and size of mini-implants for bicortical anchorage. This will result in greater stability, less deformation and fracture of the mini-implants, greater parallel expansion in the coronal plane, and greater expansion during expansion transmitted into the bone.
