Actitudes y emociones del enfermero hacia el cuidado del paciente moribundo en el servicio de emergencia

Abstract

El enfermero especialista en emergencias y desastres está capacitado para brindar una atención integral con calidad científico, técnico, humanístico especializado en pacientes críticos con alto riesgo de muerte implicando la adopción de actitud cálida para fomentar la interacción con el paciente y familia, en particular durante la etapa final de la vida. Objetivo: Analizar las actitudes y emociones del enfermero hacia el cuidado del paciente moribundo en el servicio de emergencia. Metodología: Estudio de revisión bibliográfica, se recopiló 30 artículos de los últimos 5 años a través de diferentes bases de datos: Pubmed y Scielo. Resultados: La mayoría de artículos seleccionados corresponden a la base de datos Pubmed; España y Brasil fueron los países que abarcaron el mayor número de publicaciones seleccionadas; se encontró el mayor número de artículos de diseño descriptivo, transversal y correlacional; los periodos con mayor número de publicaciones fueron entre los años 2021 y 2022; se identificaron 19 artículos (63%) señalando actitudes de acercamiento o positivas del enfermero hacia el cuidado del paciente moribundo. Por otro lado, se identificaron 11 artículos (37%) señalando actitudes de aislamiento o negativa del enfermero frente al cuidado del paciente moribundo. Se identificaron 10 artículos señalando emociones como tristeza, compasión, impotencia, frustración, fracaso, culpa, angustia, y ansiedad. Conclusión: La actitud del enfermero hacia el paciente moribundo en el servicio de emergencia es mayoritariamente de acercamiento o positiva. Las emociones del enfermero hacia el cuidado del paciente moribundo fueron tristeza, compasión, impotencia, frustración, fracaso, culpa, angustia, y ansiedad.

The nurse specialist in emergencies and disasters is trained to provide comprehensive care with scientific, technical, and humanistic quality specialized in critical patients with high risk of death, implying the adoption of a warm attitude to encourage interaction with the patient and family, particularly during the end stage of life. Objective: To analyze the attitudes and emotions of the nurse towards the care of the dying patient in the emergency service. Methodology: Bibliographic review study, 30 articles from the last 5 years were collected through different databases: Pubmed and Scielo. Results: Most of the selected articles correspond to the Pubmed database; Spain and Brazil were the countries that included the largest number of selected publications; the largest number of articles of descriptive, cross-sectional and correlational design was found; the periods with the highest number of publications were between 2021 and 2022; 19 articles (63%) were identified indicating close or positive attitudes of the nurse towards the care of the dying patient. On the other hand, 11 articles (37%) were identified indicating isolation or negative attitudes of the nurse towards the care of the dying patient. 10 articles were identified indicating emotions such as sadness, compassion, helplessness, frustration, failure, guilt, anguish, and anxiety. Conclusion: The attitude of the nurse towards the dying patient in the emergency service is mostly approaching or positive. The nurse's emotions towards the care of the dying patient were sadness, compassion, helplessness, frustration, failure, guilt, anguish, and anxiety.
