Evaluación tomográfica del cambio de posición condilar post cirugía ortognática de pacientes atendidos en un Centro Dental Docente, Lima-Perú, 2015-2024

Abstract

Introducción: La cirugía ortognática constituye una intervención eficaz para corregir discrepancias esqueléticas que afectan la función masticatoria y la estética facial. Sin embargo, este procedimiento puede alterar la posición de la articulación temporomandibular, cuya adecuada adaptación postoperatoria es fundamental para lograr estabilidad funcional. Factores como el tipo de osteotomía, el sistema de fijación empleado y los movimientos mandibulares realizados influyen en esta adaptación, haciendo necesario evaluar su impacto para optimizar los resultados clínicos. Objetivo: Evaluar tomográficamente el cambio de posición condilar post cirugía ortognática de pacientes atendidos en un Centro Dental Docente, Lima-Perú, 2015-2024. Materiales y métodos: El presente estudio será de tipo transversal, en el cual se considerarán la variable cambio de posición condilar y las covariables: tipo de deformidad dentofacial, sexo, grupo etario, lado intervenido, tipo de osteotomía y tipo de técnica de fijación. Se procederá a realizar una evaluación de las tomografías computarizadas de haz cónico pre y post quirúrgicas, previa calibración de los investigadores. Se realizará un análisis descriptivo de la variable y covariables, y un análisis bivariado mediante la prueba T de Student para muestras relacionadas, con un nivel de confianza de 95% y p<0.05. Para ello, se empleará el programa estadístico Stata v. 18.0. Conclusiones: Este estudio permitirá comprender mejor los cambios condilares post cirugía ortognática, optimizando diagnósticos, tratamientos y pronósticos en beneficio de la estabilidad funcional y esquelética del paciente.

Introduction: Orthognathic surgery is an effective intervention to correct skeletal discrepancies that compromise masticatory function and facial aesthetics. However, this procedure can alter the position of the temporomandibular joint, and proper postoperative adaptation is essential to achieve functional stability. Factors such as the type of osteotomy, the fixation system used, and the mandibular movements performed influence this adaptation, making it necessary to evaluate their impact to optimize clinical outcomes. Objective: Evaluate tomographically the condylar position changes after orthognathic surgery in patients treated at a Teaching Dental Center, Lima-Peru, 2015–2024. Materials and Methods: The present study will be a transversal design, considering the variable of condylar position change and the covariates: type of dentofacial deformity, sex, age group, operated side, type of osteotomy, and type of fixation technique. Pre- and postoperative cone-beam computed tomography scans will be evaluated following investigator calibration. A descriptive analysis of the variable and covariates will be performed, along with a bivariate analysis using the paired Student's t-test, with a 95% confidence level and p<0.05. Stata v. 18.0 statistical software will be used. Conclusions: This study will contribute to a better understanding of condylar changes following orthognathic surgery, optimizing diagnosis, treatment, and prognosis to enhance the patient's functional and skeletal stability.
