Abordaje inmunohematológico de la Enfermedad Hemolítica del recién nacido por incompatibilidad ABO

Abstract

En los años recientes la incompatibilidad ABO ha emergido como causa significativa de la Enfermedad Hemolítica del recién nacido (EHRN), conocida como Eritroblastosis fetalis, lo que conlleva a morbilidad y mortalidad neonatal; la patogénesis de esta enfermedad involucra el pasaje a través de la placenta de los anticuerpos IgG anti-A, anti-B y anti-A, B resultando en lisis de los hematíes neonatales. La EHRN, si no es detectada a tiempo y tratada oportunamente, puede llegar a complicaciones fatales. La EHRN por incompatibilidad ABO puede estar acompañada de diferentes grados de signos clínicos: anemia, ictericia, hiperbilirrubinemia, hepatomegalia y otras complicaciones, incluso la muerte. El tratamiento prenatal incluye parto prematuro, transfusión sanguínea intrauterina, etc., y el tratamiento posnatal incluye fototerapia, exanguíneo transfusión, etc. Entre las pruebas inmunohematológicas actualmente utilizadas para el diagnóstico oportuno de la EHRN, tenemos los grupos sanguíneos ABO y Rh, la prueba de Coombs directa e indirecta, la titulación de anticuerpos IgG anti-A y anti-B, y en algunos casos la técnica de elución. Según la revisión realizada, ninguna de ellas ofrece por sí sola un diagnóstico definitivo de la EHRN por incompatibilidad ABO, si no, por el contrario, muchas de ellas realizadas en conjunto brindan una mayor posibilidad al diagnóstico oportuno. El objetivo de este estudio descriptivo es revisar y analizar los diferentes abordajes sobre la EHRN en los protocolos actuales.

In recent years, ABO incompatibility has emerged as a significant cause of Hemolytic Disease of the Newborn (HDN), known as Erythroblastosis fetalis, leading to neonatal morbidity and mortality; the pathogenesis of this disease involves the passage through the placenta of anti-A, anti-B and anti-A, B IgG antibodies resulting in lysis of neonatal red blood cells. EHRN, if not detected in time and treated promptly, can lead to fatal complications. HDN due to ABO incompatibility may be accompanied by different degrees of clinical signs: anemia, jaundice, hyperbilirubinemia, hepatomegaly and other complications, even death. Prenatal treatment includes premature delivery, intrauterine blood transfusion, etc., and postnatal treatment includes phototherapy, exchange transfusion, etc. Among the immunohematological tests currently used for the timely diagnosis of HDN, we have the ABO and Rh blood groups, the direct and indirect Coombs test, the titration of anti-A and anti-B IgG antibodies, and in some cases the elution technique. According to the review, none of them alone offers a definitive diagnosis of HDN due to ABO incompatibility; on the contrary, many of them performed together.
