Relación entre la enfermedad hepática esteatósica asociada a disfunción metabólica y el carcinoma hepatocelular en población adulta del continente americano: revisión sistemática

dc.description.abstractEl carcinoma hepatocelular (CHC) es el tumor maligno primario más frecuente del hígado y representa un importante problema de salud pública debido a su alta incidencia y mortalidad. Las principales causas son las hepatitis virales, el consumo de alcohol y, de forma creciente, la enfermedad hepática esteatósica asociada a disfunción metabólica (MASLD). Esta condición, vinculada a la obesidad y al síndrome metabólico, puede evolucionar a su forma más grave, la esteatohepatitis (MASH), que aumenta significativamente el riesgo de CHC, incluso en ausencia de cirrosis. El objetivo de este estudio es evaluar la relación entre MASLD y CHC en población adulta del continente americano, mediante una revisión sistemática de la literatura publicada entre 2015 y 2025. Se realizó una búsqueda exhaustiva en bases de datos internacionales y regionales, utilizando descriptores normalizados y términos libres en español e inglés. Se incluyeron estudios experimentales y observacionales excluyendo reportes de casos y estudios sin acceso al texto completo. El análisis contempló la descripción de las medidas de riesgo de MASLD, CHC y evaluación de la calidad de la evidencia con el instrumento Newcastle–Ottawa Scale. Se incluyeron estudios con tamaños muestrales entre 102 y más de 1,8 millones de participantes. Los hazard ratios (HR) reportados variaron desde 1,60 (IC 95%: 1,40–1,83) hasta 7,62 (IC 95%: 5,76–10,09). Los riesgos relativos (RR) oscilaron entre 0,20 y 4,01, mostrando que en poblaciones con enfermedad metabólica avanzada el riesgo puede incrementarse notablemente. La incidencia fue baja en población del continente americano, pero mayor en pacientes con cirrosis de etiología metabólica. La evidencia revisada indica que MASLD constituye un factor de riesgo para el desarrollo de CHC en el continente americano, resaltando la necesidad de fortalecer estrategias de prevención y vigilancia.spa
dc.description.abstractHepatocellular carcinoma (HCC) is the most common primary malignant tumor of the liver and represents a major public health concern due to its high incidence and mortality rates. The main causes of HCC include viral hepatitis, alcohol consumption, and, increasingly, metabolic dysfunction–associated steatotic liver disease (MASLD). This condition, which is linked to obesity and metabolic syndrome, may progress to its more severe form, metabolic dysfunction–associated steatohepatitis (MASH), significantly increasing the risk of HCC, even in the absence of cirrhosis. The aim of this study was to evaluate the relationship between MASLD and HCC in the adult population of the American continent through a systematic review of the literature published between 2015 and 2025. A comprehensive search was conducted in international and regional databases using standardized descriptors and free-text terms in both Spanish and English. Experimental and observational studies were included, while case reports and studies without full-text access were excluded. The analysis included the description of risk measures for MASLD and HCC, as well as the assessment of evidence quality using the Newcastle–Ottawa Scale. Studies with sample sizes ranging from 102 to more than 1.8 million participants were included. Reported hazard ratios (HRs) ranged from 1.60 (95% CI: 1.40–1.83) to 7.62 (95% CI: 5.76–10.09). Relative risks (RRs) ranged from 0.20 to 4.01, showing that the risk may increase substantially in populations with advanced metabolic disease. The incidence of HCC was low in the American population overall but was higher among patients with cirrhosis of metabolic etiology. The reviewed evidence indicates that MASLD constitutes a risk factor for the development of HCC in the American continent, highlighting the need to strengthen prevention and surveillance strategies.eng
