Utilidad de células CAR-T en el tratamiento de leucemia mieloide crónica

Abstract

La leucemia mieloide crónica, es una enfermedad hematológica que genera un deterioro prolongado, disminuye los años útiles de vida y que gracias al uso de tratamientos convencionales puede brindar más años de sobrevida, pero no ofrece una cura y en muchos casos generan resistencia e intolerancia al tratamiento. Un nuevo enfoque terapéutico denominado terapia de células T con receptores de antígenos quiméricos (CAR-T), es un procedimiento innovador que en ensayos in vivo e in vitro recientes, ha demostrado ser una opción segura y duradera para el tratamiento de la leucemia mieloide crónica (LMC). Este trabajo tiene por objetivo analizar el contexto de aplicación de la terapia CAR-T en la LMC, describir diversos estudios internacionales que demuestran la viabilidad de la terapia en pacientes con LMC en sus diferentes fases y describir el contexto peruano frente a la tecnología sanitaria presente, así como la situación epidemiológica que permita aplicar la terapia CAR-T. Para esto, se llevó a cabo una rigurosa búsqueda bibliográfica en diversas bases de datos como Medline (PubMed), Scopus y SciELO Data. La revisión permitió concluir que, la terapia CAR-T es beneficiosa para pacientes con LMC, teniendo una mayor tasa de éxito frente al tratamiento convencional, sin embargo, aún no se aplica en el contexto peruano.

Chronic myeloid leukemia is a hematological disease that causes prolonged deterioration, reduces the useful years of life and, thanks to the use of conventional treatments, can provide more years of survival, but it does not offer a cure and in many cases generates resistance and intolerance to treatment. A new therapeutic approach called chimeric antigen receptor T-cell therapy (CAR-T), is an innovative procedure that in recent in vivo and in vitro trials, has proven to be a safe and long-lasting option for the treatment of chronic myeloid leukemia (CML). This work aims to analyze the context of application of CAR-T therapy in CML, describe various international studies that demonstrate the viability of the therapy in patients with CML in its different phases and describe the Peruvian context in relation to current health technology, as well as the epidemiological situation that allows the application of CAR-T therapy. For this, a rigorous bibliographic search was carried out in various databases such as Medline (PubMed), Scopus and SciELO Data. The review concluded that CAR-T therapy is beneficial for patients with CML, having a higher success rate compared to conventional treatment, however, it is not yet applied in the Peruvian context.
