Enfermedad hepática esteatósica asociada a disfunción metabólica en pacientes con síndrome de ovario poliquístico: revisión narrativa

dc.description.abstractLa asociación entre el síndrome de ovario poliquístico (SOP) y la enfermedad hepática esteatósica asociada a disfunción metabólica (MASLD) constituye un problema clínico relevante por su base fisiopatológica común y su elevada carga cardiometabólica. Esta revisión narrativa sintetiza 35 estudios publicados entre 2020 y 2025, abarcando evidencia clínica, genética y bioinformática. Los hallazgos muestran que la resistencia a la insulina, el hiperandrogenismo, la adiposidad visceral y la inflamación de bajo grado conforman el eje hepato-ovárico que vincula ambas condiciones. La prevalencia de MASLD en mujeres con SOP varía entre 26 % y 77 %, con mayor riesgo en fenotipos hiperandrogénicos y en mujeres con obesidad central, dislipidemia o HOMA-IR elevado. La coexistencia de SOP y MASLD configura un fenotipo de alto riesgo, asociado con síndrome metabólico, diabetes tipo 2, alteraciones reproductivas y progresión hacia esteatohepatitis y fibrosis en subgrupos de mayor riesgo. La evaluación no invasiva debe diferenciar la identificación de esteatosis de la estratificación de fibrosis. Herramientas como FLI, HSI, ecografía hepática o CAP pueden orientar la detección o estimación de esteatosis, mientras que FIB-4, NFS y la medición de rigidez hepática mediante elastografía transitoria se orientan a la estratificación de fibrosis. Las intervenciones basadas en estilo de vida presentan los beneficios más consistentes, mientras que la evidencia farmacológica específica sigue siendo limitada. Se resalta la necesidad de un abordaje clínico integral, evaluación hepatometabólica dirigida y escalonada, e investigación orientada a mejorar la detección y manejo de esta comorbilidad emergente.spa
dc.description.abstractThe association between polycystic ovary syndrome (PCOS) and metabolic dysfunction-associated steatotic liver disease (MASLD) represents a clinically relevant issue due to their shared pathophysiological basis and high cardiometabolic burden. This narrative review synthesizes 35 studies published between 2020 and 2025, including clinical, genetic, and bioinformatic evidence. Findings show that insulin resistance, hyperandrogenism, visceral adiposity, and low-grade inflammation constitute the hepato-ovarian axis linking both conditions. The prevalence of MASLD in women with PCOS ranges from 26% to 77%, with higher risk among hyperandrogenic phenotypes and women with central obesity, dyslipidemia, or elevated HOMA-IR. The coexistence of PCOS and MASLD defines a high-risk phenotype associated with metabolic syndrome, type 2 diabetes, reproductive dysfunction, and progression to steatohepatitis and fibrosis in higher-risk subgroups. Non-invasive assessment should distinguish steatosis identification from fibrosis stratification. Tools such as FLI, HSI, liver ultrasound, or CAP may support the detection or estimation of hepatic steatosis, whereas FIB-4, NFS, and liver stiffness measurement by transient elastography are mainly oriented toward fibrosis risk stratification. Lifestyle-based interventions show the most consistent benefits, while phenotype-specific pharmacological evidence remains limited. These findings highlight the need for integrated clinical management, targeted and stepwise hepatometabolic assessment, and further research aimed at improving the detection and management of this emerging comorbidity.eng
