Anticuerpos sensibilizantes en la incompatibilidad ABO materno – fetal a través del test de Coombs directo y elución con reactivo comercial en un hospital general de Huancayo – 2019

Abstract

La ocurrencia de enfermedad hemolítica por incompatibilidad ABO materno fetal es un problema de salud con variada frecuencia de reporte. La incompatibilidad puede ser detectada por múltiples pruebas de laboratorio. No obstante, no todas las pruebas tienen la misma capacidad de detección. Por ejemplo, un porcentaje mínimo de casos es positivo para la prueba de antiglobulina directa, mientras que un gran porcentaje tiene resultados negativos para la misma prueba. Es posible encontrar pequeñas cantidades de IgG unidas a la membrana de los eritrocitos del recién nacido y al realizar la técnica de elución con reactivo comercial, los casos previamente identificados como negativos podrían tener un resultado positivo. Por tanto, es necesario valorar el uso complementario de ambas pruebas con el objetivo de correctamente caracterizar los casos de incompatibilidad. Objetivo: Determinar la frecuencia de anticuerpos sensibilizantes detectados por el test de Coombs directo y elución con reactivo comercial en casos de incompatibilidad ABO materno – fetal en un hospital general de Huancayo durante el 2019. Material y método: Estudio de tipo observacional, descriptivo de corte transversal. Se incluirá todos los registros de recién nacidos con incompatibilidad ABO materno fetal (grupo sanguíneo del recién nacido es A, B, AB y el grupo sanguíneo de la madre O). Se recolectará información de las diversas pruebas ejecutadas y luego se caracterizará la ocurrencia y características de los casos de incompatibilidad. Las variables categóricas serán descritas utilizando frecuencias absolutas y relativas. El análisis de concordancia se realizará a través del índice de Kappa, y todo proceso será realizado en Stata v15.

Therefore, it is necessary to assess the complementary use of both tests in order to correctly characterize cases of incompatibility. Objective: To determine the frequency of sensitizing antibodies detected by the direct Coombs test and elution with a commercial reagent in cases of maternal-fetal ABO incompatibility in a general hospital in Huancayo during 2019. Material and method: Observational, descriptive study. cross. All records of newborns with maternal-fetal ABO incompatibility (newborn's blood group is A, B, AB and mother's blood group O) will be included. Information will be collected from the various tests carried out and then the occurrence and characteristics of the incompatibility cases will be characterized. Categorical variables will be described using absolute and relative frequencies. The concordance analysis will be carried out through the Kappa index, and the entire process will be carried out in Stata v15.
