Desempeño diagnóstico de biomarcadores séricos: FIB-4, APRI, NAFLD score versus biopsia hepática en la valoración de fibrosis avanzada en pacientes obesos atendidos en la Clínica Avendaño en los años 2016 a 2018

Abstract

Objetivos: describir el desempeño diagnóstico de los tests séricos: FIB-4, APRI, NAFLD score versus la biopsia hepática en la valoración de fibrosis avanzada en pacientes obesos atendidos en la clínica Avendaño del 2016 a 2018. Materiales y métodos: es un estudio de tipo observacional, transversal, analítico y retrospectivo de pacientes obesos evaluados para ser sometidos a cirugía bariátrica. Como parte del protocolo estandarizado de la clínica, los pacientes también fueron sometidos a biopsia hepática previa firma de consentimiento para estudiar el compromiso hepático causado por la obesidad. En esta población de pacientes se calcularon las pruebas APRI, FIB-4 y NAFLD score y se determinó su validez, seguridad y razón de probabilidad. Resultados: Se evaluaron 203 pacientes obesos, hallando que la media de IMC fue 37.99, 41 pacientes con diabetes mellitus; la media de las plaquetas fue de 302.23 U/mm3, de la TGP fue de 53.73 U/L, de la TGO fue de 35.58 U/L, de la albúmina fue de 4.38 gr/dL y de la glucosa fue de 92.59 mg/dL. 19.7 % presentaron fibrosis hepática avanzada. La prueba de FIB-4 reportó una sensibilidad de 2.5 %, especificidad de 98.39 %, valor predictivo positivo (VPP) de 50 % y valor predictivo negativo (VPN) de 80.58 %. La prueba de APRI reportó una sensibilidad de 10 %, especificidad de 98.16 %, VPP de 57.14 % y VPN de 81.63 %. La prueba de NAFLD reportó una sensibilidad de 10%, especificidad de 99.39 %, VPP de 80% y VPN de 81.82 %. Conclusión: los tests no invasivos APRI, FIB-4 y NAFLD score index son altamente específicos, pero poco sensibles.

Objectives: To describe the diagnostic performance of serum tests: FIB-4, APRI, NAFLD score versus liver biopsy in the assessment of advanced fibrosis in obese patients treated at the Avendaño clinic from 2016 to 2018. Materials and methods: This is an observational, cross-sectional, analytical and retrospective study of obese patients evaluated to undergo gastric sleeve surgery. As part of the clinic's standardized protocol, patients also underwent liver biopsy after signing a consent to study liver involvement caused by obesity. In this patient population, the APRI, FIB-4 and NAFLD score tests were calculated and their validity, safety and likelihood ratio were determined. Results: A total of 203 obese patients were evaluated, finding that the mean BMI was 37.99, 41 patients with diabetes mellitus; The mean platelet count was 302.23 U/mm3, the SPT was 53.73 U/L, the SGT was 35.58 U/L, the albumin was 4.38 gr/dL and the glucose was 92.59 mg/dL. 19.7% presented advanced liver fibrosis. The FIB-4 test reported a sensitivity of 2.5%, specificity of 98.39%, positive predictive value (PPV) of 50% and negative predictive value (NPV) of 80.58%. The APRI test reported a sensitivity of 10%, specificity of 98.16%, PPV of 57.14% and NPV of 81.63%. The NAFLD test reported a sensitivity of 10%, specificity of 99.39%, PPV of 80% and NPV of 81.82%. Conclusion: The noninvasive tests APRI, FIB-4 and NAFLD score index are highly specific, but not very sensitive.
