Evaluación de la distancia interincisiva post expansión rápida maxilar con anclaje convencional o esquelético

Abstract

La deficiencia transversal maxilar (DTM) se define como la alteración en la armonía de los anchos esqueléticos de los huesos maxilares que tendrá como alternativa de tratamiento la expansión rápida maxilar (ERM). La ERM es un tratamiento ortopédico, que puede usar anclaje dentario, esquelético o ambos; genera un aumento transversal del maxilar y remodelación de las suturas circunmaxilares en especial la sutura media palatina y presenta mayor diferenciación en la distancia interincisiva (ID). El objetivo del estudio será evaluar mediante tomografías computarizadas de haz cónico (TCHC) la ID post ERM con anclaje convencional o esquelético. El presente estudio es de tipo observacional, descriptivo, longitudinal y retrospectivo. El procedimiento consistirá en la ubicación del ID en las imágenes DICOM, se orientará el volumen tomográfico usando una línea que conectará los bordes inferiores infraorbitarios en la vista frontal y el plano horizontal de Frankfort en la vista sagital. Se realizarán las medidas en la sección cigomático axial; los valores lineales se obtendrán midiendo la distancia interincisiva incisal, distancia interincisiva medial y distancia interinsiva cervical de las TCHC en T0 (pre- expansión) y T1 (post-expansión). La diferencia entre las mediciones T0 y T1 indicará la variación en las dimensiones lineales del ID. Se empleará estadística descriptiva (media y desviación estándar)para los dos grupos y estadística analítica.

Transverse maxillary deficiency (MTD) is an alteration in the harmony of theskeletal widths of the maxillary bones, for which rapid maxillary expansion (RM) is a treatment option. RM is an orthopedic treatment that can use dental, skeletal, or both anchorages; it generates a transverse increase in maxillary width and remodels the circummaxillary sutures, especially the midpalatal suture, and resultsin greater differentiation in the interincisal distance (ID). The objective of this study is to evaluate the ID post-RM with conventional or skeletal anchorage using conebeam computed tomography (CBCT). This is an observational, descriptive, longitudinal, and retrospective study. The procedure will consist of locating the ID on DICOM images. The tomographic volume will be oriented using a line connecting the lower infraorbital margins in the frontal view and the Frankfort horizontal plane in the sagittal view. Measurements will be taken in the zygomaticaxial section. Linear values will be obtained by measuring the incisal interincisal distance, medial interincisal distance, and cervical interincisal distance of the TCHC at T0 (pre-expansion) and T1 (post-expansion). The difference between theT0 and T1 measurements will indicate the variation in the linear dimensions of the ID. Descriptive statistics (mean and standard deviation) will be used for bothgroups, as well as analytical statistics.
