Cáncer testicular y plan de capacitación para su detección temprana

Abstract

El cáncer testicular de células germinales no seminomatoso (CTCGNS) es un cáncer poco frecuente, representa el 1 % de todos los cánceres; sin embargo, su comportamiento suele ser muy agresivo, y presentarse en varones jóvenes entre 20 y 40 años. El lugar más frecuente de metástasis son los ganglios retroperitoneales, en los CTCGNS, de persistir una masa residual retroperitoneal postquimioterapia, esta debe resecarse. Las histopatologías más frecuentes son necrosis/fibrosis, teratoma y cáncer viable. No existe un método de predecir que histopatología correspondería a la masa residual, quedando como única alternativa la linfadenectomía retroperitoneal. Existen algunos reportes iniciales, que una reducción importante de la masa residual retroperitoneal postquimioterapia, podría corresponder a necrosis/fibrosis; de esta manera, evitar una cirugía tan agresiva y con potenciales complicaciones y secuelas en un varón joven.

Non-seminomatous germ cell testicular cancer (NSGCT) is a rare cancer, accounting for 1% of all cancers; however, its behavior is usually highly aggressive and presents in young men between 20 and 40 years of age. The most common site of metastasis is the retroperitoneal lymph nodes. In NSGCT, if a residual retroperitoneal mass persists postchemotherapy, it must be resected. The most common histopathologies are necrosis/fibrosis, teratoma, and viable cancer. Thereis no method to predict which histopathology would correspond to the residual mass, leaving retroperitoneal lymphadenectomy as the only alternative. There are some initial reports that a significant reduction in the residual retroperitoneal mass postchemotherapy could correspond to necrosis/fibrosis; thus, such an aggressive surgery, with potential complications and sequelae in a young man, could be avoided.
