Intervención de la enfermera en el uso de técnicas de alimentación para la transición de vía enteral a la vía oral en neonatos prematuros en la unidad de cuidados intensivos neonatales

Abstract

La excelencia de la lactancia materna directa sobre cualquier otra técnica de alimentación oral es incuestionable. Sin embargo, para los RNPT la separación prolongada de su madre, la inmadurez en la función de succión así como la falta de coordinación entre la succión, la deglución y respiración; el estado de alerta a corto plazo y la debilidad de los músculos orofaciales hacen que las ofertas de dieta oral sean inseguras y contribuyen a la dificultad para comenzar y mantener la lactancia materna. La forma en que se administra la leche por vía oral a los bebés prematuros que no pueden amamantar puede influir en su transición alimentaria. Objetivos: Identificar las diferentes técnicas de alimentación alternativas al uso del biberón para la transición de la vía enteral a la vía oral en RNPT en la UCI neonatal. Metodología: Se realizó una revisión bibliográfica, descriptivo. El estudio contó con un total de 33 artículos científicos de bases de datos PUBMED 46%(15), SCIELO 15%(5), REVISTAS PEDIATRICAS 9%(3), REV. BRASILERAS 6%(2) y ELSEVIER, entre otros, cada una con el 3%(1) de diseño cuantitativo, cualitativo y ensayos clínicos. Conclusiones: Se evidenció que los RNPT con o sin antecedentes de complicaciones médicas importantes, les surgen dificultades cuando se someten a ésta transición. Ésta revisión bibliográfica contribuyó a elucidar las diferentes técnicas y perspectivas teóricas de alimentación; sobre porqué ofrecer una dieta mediante vaso-taza, succión al dedo, jeringa, cuchara, sonda al pecho y no usando biberón.

The excellence of direct breastfeeding over any other oral feeding technique is unquestionable. However, for RNPT the prolonged separation from their mother, the immaturity in the sucking function as well as the lack of coordination between sucking, swallowing and breathing; short-term alertness and weakness of the orofacial muscles make oral diet offers unsafe and contribute to difficulty in initiating and maintaining breastfeeding. The way milk is given by mouth to premature babies who cannot breastfeed can influence their feeding transition. Objectives: To identify the different alternative feeding techniques to the use of the bottle for the transition from the enteral route to the oral route in RNPT in the neonatal ICU. Methodology: A descriptive bibliographical review was carried out. The study had a total of 33 scientific articles from databases PUBMED 46%(15), SCIELO 15%(5), PEDIATRIC JOURNALS 9%(3), REV. BRASILERAS 6%(2) and ELSEVIER, among others, each with 3%(1) of quantitative and qualitative design and clinical trials. Conclusions: It was evidenced that RNPT with or without a history of important medical complications, difficulties arise when they undergo this transition. This literature review contributed to elucidate the different feeding techniques and theoretical perspectives; about why offer a diet through cup-cup, finger suction, syringe, spoon, chest tube and not using a bottle.
