Postura craneocervical pre y post tratamiento ortodóntico quirúrgico de pacientes con maloclusión clase III atendidos en un Centro Dental Docente, Lima-Perú, 2015-2024

Abstract

Introducción: La postura craneocervical es el sistema de equilibrio entre el cráneo y la porción superior de la columna vertebral. Alteraciones en la postura afectan la posición de la mandíbula, la oclusión, el componente neuromuscular y la articulación temporomandibular. En pacientes con maloclusión de clase III con prognatismo mandibular hay un aumento de las curvas vertebrales y flexión craneal, lo cual aumenta la probabilidad de patologías. La corrección quirúrgica cambia la posición mandibular, para así devolver el equilibrio anatómico y funcional craneocervical. Objetivo: Determinar la postura craneocervical pre y post tratamiento ortodóntico quirúrgico de pacientes con maloclusión Clase III atendidos en Centro Dental Docente, Lima -Perú, 2015-2024. Materiales y métodos: El estudio será transversal. La población estará conformada por 175 radiografías cefalométricas laterales de pacientes con maloclusión de clase III atendidos en el Centro Dental de la Universidad Peruana Cayetano Heredia, Lima – Perú, 2015 -2024; la muestra final estará conformada por 142 casos. Las variables serán: postura craneocervical, tratamiento ortodóntico quirúrgico en pacientes de clase III, sexo, edad y tiempo. Se realizará la calibración inter e intra examinador con el Análisis de Rocabado para evaluar la postura. Se realizará el trazado pre y post tratamiento de dos grupos: el primero con tratamiento ortodóntico quirúrgico y el segundo solo con tratamiento ortodóntico compensatorio (grupo control). Se realizará un análisis descriptivo, usando la prueba de Chi-cuadrado al 95%. Conclusiones: Este estudio resalta la postura craneocervical como elemento de gran importancia en el diagnóstico y planificación del tratamiento óptimo para un enfoque integral.

Introduction: Craniocervical posture is the balance system between the skull and the upper spine. Alterations in posture affect the position of the mandible, occlusion, the neuromuscular component, and the temporomandibular joint. In patients with Class III malocclusion and mandibular prognathism, there is increased vertebral curves and cranial flexion, which increases the likelihood of pathologies. Surgical correction changes the mandibular position, thereby restoring anatomical and functional balance of the craniocervical region. Objective: To determine the craniocervical posture before nd after surgical orthodontic tretament of patients with class III malocclusion treated at the Teaching Dental Center, Lima – Perú, 2015– 2024. Materials and methods: This study will be cross-sectional. The population will consist of 175 lateral cephalometric radiographs of patient with clas III malocclusion treated at the Dental Center of the Cayetano Heredia Peruvian University, Lima – Perú, from 2015 to 2024. The final sample will consist of 142cases. The variables will be: craniocervical posture, surgical orthodontic treatment in class III patients, sex, age and time. The calibration will be done inter and intra examiner using Rocabado Analysis to evaluate craniocervical posture. Pre and posttreatment tracings will be performed in two groups: the first with surgical orthodontic treatment and the second with only compensatory orthodontic treatment (control group). A descriptive análisis will be performed using the 95% chi-square test. Conclusions: This study highlights craniocervical posture as a key element in the diagnosis and planning of optimal treatment for a comprehensive approach.
