Restricción física en pacientes internados en los servicios de emergencia: una mirada de enfermería

Abstract

Introducción: En los servicios de emergencia se observa una considerable cantidad de pacientes que muestra algún tipo de desorden de conducta y una de las prácticas que más se utilizan es la restricción física a pesar que existe una carencia de un marco legislativo que proteja al paciente y al cuidador. Adicional a ello, es que este tipo de proceder no está siendo registrada en las historias clínicas por el personal como indicación de parte del médico, el tipo de contención, las causas por las que se indicó, el tiempo, cada que tiempo se está revisando y el consentimiento informado. Objetivo: Analizar la ejecución de las restricciones físicas como procedimiento en los servicios de emergencia. Metodología: el presente trabajo es una revisión bibliográfica descriptiva para ello se recopilaron 25 artículos. Considerando los criterios de inclusión como las fechas de publicación del 2017 hasta el 2022, o sean de idioma español o inglés. Los buscadores que se utilizaron fueron: Scielo, Elsevier, Redalyc Pub Med y Dialnet. Se utilizaron los buscadores boléanos AND, OR y NOT. Conclusiones: Si bien es cierto la ejecución de la restricción física constituye una terapia prescrita por el médico y ejecutada por el personal de enfermería y con el apoyo del personal técnico con la finalidad de la seguridad del paciente y prevención de eventos adversos, pero según algunos estudios nos dice que no contamos con protocolos establecidos que respalden este actuar.

Introduction: In emergency services there is a considerable number of patients who show some type of conduct disorder and one of the practices that is most used is physical restraint despite the fact that there is a lack of a legislative framework that protects the patient and to the caregiver. In addition to this, this type of procedure is not being recorded in the medical records by the staff as an indication by the doctor, the type of containment, the reasons for which it was indicated, the time, how often it is being reviewed and informed consent. Objective: Analyze the execution of physical restraints as a procedure in emergency services. Methodology: the present work is a descriptive bibliographic review, for which 25 articles were collected. Considering the inclusion criteria as the publication dates from 2017 to 2022, or whether they are Spanish or English. The search engines used were: Scielo, Elsevier, Redalyc Pub Med and Dialnet. The Boolean search engines AND, OR and NOT were used. Conclusions: Although it is true that the execution of physical restraint constitutes a therapy prescribed by the doctor and executed by the nursing staff and with the support of the technical staff with the purpose of patient safety and prevention of adverse events, but according to some Studies tell us that we do not have established protocols that support this act.
