Estrategias del profesional de enfermería perioperatoria en la prevención de infecciones del sitio quirúrgico

Abstract

Infección del sitio quirúrgico (ISQ), problema latente, alarmante a nivel mundial (15% al 30% de las infecciones nosocomiales) eleva la morbimortalidad, estancia hospitalaria y costo de tratamiento. El profesional de enfermería juega un papel trascendental en su prevención. Objetivo: Identificar las estrategias del profesional de enfermería perioperatoria a fin de prevenir las ISQ. Metodología: Descriptivo, retrospectivo; revisión de artículos (2019 a 2023). Resultados: Estrategias en el preoperatorio: baño del paciente, profilaxis antibiótica, rasurado de la zona operatoria solo si amerita, lavado de zona operatoria, lavado quirúrgico de manos, verificación de esterilidad de instrumentales; recomendado por el 91,6%. En el intraoperatorio se debe: mantener al paciente en normoglicemia, normotermia, normoxemia y reducir al mínimo el tránsito de personal dentro del quirófano sugerido por el 85,8%. En el postoperatorio: mantener en normoglicemia, normoxemia, normotermia al paciente, profilaxis antibiótico posoperatorio, en cirugías contaminadas, mantenimiento de la herida quirúrgica con vendaje de 24 a 48 horas del posoperatorio, fluidoterapia controlada, cuidados de drenajes; recomendado por el 85,7%. Conclusiones: Estrategias del experto en el cuidado perioperatorio para evitar las infecciones del punto operable deben ser aplicadas durante el preoperatorio, intraoperatorio y postoperatorio del paciente.

Methodology: Descriptive, retrospective; review of articles (2019 to 2023). Results: Preoperative strategies: patient bathing, antibiotic prophylaxis, shaving of the operative area only if necessary, washing of the operative area, surgical hand washing, verification of sterility of instruments; recommended by 91.6% of scientists. n the intraoperative period, the patient should be kept in normoglycemia, normothermia, normoxemia and reduce to a minimum the transit of personnel inside the operating room, as suggested by 85.8%. In the postoperative period: maintain the patient in normoglycemia, normoxemia, normothermia, postoperative antibiotic prophylaxis, in contaminated surgeries, maintenance of the surgical wound with bandage from 24 to 48 hours postoperatively, controlled fluid therapy, drainage care; recommended by 85.7%. Conclusions: Expert strategies in perioperative care to avoid infections of the operable point should be applied during the preoperative, intraoperative and postoperative period of the patient.
