Antecedentes: En el Perú, la prevalencia estimada de la coinfección entre el virus de la inmunodeficiencia humana (VIH) y el virus linfotrópico T humano (HTLV) es 2.9% en la población general. Actualmente, existe controversia en el impacto de la coinfección en la progresión clínica y mortalidad, constituyendo un importante problema de salud pública. Objetivo: Comparar características sociodemográficas, clínicas y de mortalidad entre pacientes con la coinfección VIH/HTLV e infección por VIH. Material y métodos: Cohorte retrospectiva de pacientes mayores igual de 18 años incluidos a las cohortes de VIH y HTLV del Instituto de Medicina Tropical Alexander von Humboldt en el periodo 1989 - 2019. A cada paciente coinfectado se asignó aleatoriamente dos pacientes infectados con VIH con características similares: sexo, edad al ingreso (+/-5 años) y año de diagnóstico de VIH. Los datos fueron recolectados a partir de fichas epidemiológicas de las cohortes. Resultados: En la muestra total, la mediana de la edad fue 41.5 años (RQ=34-51 años), la mayoría fueron hombres (61%), solteros (44.4%), heterosexuales (71%), nacidos en Lima (58%), con un nivel educativo de secundaria (55.6%) y recibieron tratamiento antirretroviral (91.4%). La tasa de mortalidad global fue 13.6 por 100 personas y el tiempo de supervivencia fue significativamente mayor en pacientes coinfectados (mediana=14.19 años) frente a los infectados con VIH (mediana=23.83 años) (p<0.001). Conclusiones: Los pacientes coinfectados presentaron una supervivencia significativamente menor a comparación de los infectados con VIH; sin embargo, es fundamental realizar estudios prospectivos a mayor escala para conocer su verdadero impacto.

Objective: To compare socio-demographic and clinical characteristics, as well as mortality rates between HIV-infected and HIV/HTLV co-infected patients. Methods and Materials: Retrospective cohort of patients aged 18 years and older belonging to the HIV and HTLV cohort of the Alexander von Humboldt Institute of Tropical Medicine in the period 1989-2019. Each co-infected patient was randomly assigned two HIV-infected patients with similar characteristics: sex, age at admission (+/-5 years) and year of HIV diagnosis. Data were collected from epidemiological records of each cohort. Results: In the total sample, the median age was 41.5 years (RQ=34-51 years), the majority were male (61%), single (44.4%), heterosexual (71%), born in Lima (58%), with a secondary school education (55.6%) and receiving antiretroviral treatment (91.4%). The overall mortality rate was 13.6 per 100 persons and survival time for co-infected patients (median=14.19 years) was significantly shorter than in HIV-infected patients (median=23.83 years) (p<0.001). Conclusion: Co-infected patients had significantly lower survival compared to HIV-infected patients; however, larger-scale prospective studies are essential to understand the true impact.
