Objetivo: Determinar los factores de riesgo del paciente, de la enfermedad y el tratamiento, asociados al abandono al tratamiento para TB en la población peruana Materiales y métodos: Se realizó un estudio de cohorte retrospectiva, usando la información del sistema de vigilancia epidemiológica del Perú entre los años 2013 y 2015. Se utilizaron curvas Kaplan-Meier para evaluar patrones del riesgo de abandono. Se utilizó el modelo de Cox de efectos mixtos para el análisis multivariado. Resultados: Se incluyeron 28 521 registros de pacientes mayores de 15 años. la proporción acumulada de abandono al tratamiento de TB sensible fue 10.5% y de 42.5% para TB resistente. El riesgo de abandono fue mayor cerca al cambio de fase del tratamiento y se observó una variabilidad del riesgo según departamento. Independientemente del perfil de resistencia, el abandono previo (HR: 4,91 [4.37 – 5.51] TB sensible y HR 2.09 [1.69-2.58] TB resistente) y el consumo de drogas (HR: 3.27 [ 2.89-369] TB sensible y HR: 2.16 [1.71-2.75] TB resistente) fueron los dos factores de mayor riesgo para abandono. Para TB sensible un factor adicional fue la comorbilidad con VIH en el género masculino (HR: 3.24 [2.27, 4.61]) y para TB MDR el género masculino (HR: 1.49 [1.20, 1.85]). Conclusiones: El riesgo de abandono fue mayor en el momento cercano al cambio de fase del tratamiento. El abandono previo, el consumo de drogas, la infección por el VIH se asoció a mayor probabilidad de abandono en pacientes TBC sensible y resistente.Objective: To determine the patient, disease and treatment risk factors associated with TB treatment Interruption in the Peruvian population. The risk of treatment Interruption was higher near the change of treatment phase and there was variability of risk by department. Regardless of the resistance profile, previous treatment Interruption (HR: 4,91 [4.37 – 5.51] sensitive TB and HR 2.09 [1.69-2.58] resistant TB) and drug use (HR: 3.27 [ 2.89-369] sensitive TB and HR: 2.16 [1.71-2.75]) were the two highest risk factors for treatment Interruption. For sensitive TB an additional factor was HIV comorbidity in male gender (HR: 3.24 [2.27, 4.61]) and for MDR TB male gender (HR: 1.49 [1.20, 1.85]).
