ANTECEDENTES: Los pacientes con periodontitis que sufren de migración patológica de los dientes, necesitan un tratamiento que controle la periodontitis y devuelva una posición adecuada de los dientes. El análisis de biomarcadores relacionados al desarrollo de la periodontitis puede reflejar molecularmente los cambios estructurales que ocurren en los dientes con soporte periodontal reducido bajo fuerzas ortodóncicas. El objetivo del presente estudio fue cuantificar los niveles de IL-6, IL-17A, MMP-8, RANKL y OPG en el fluido gingival crevicular (FCG) de pacientes periodontalmente comprometidos, sometidos a tratamiento de ortodoncia. METODOLOGÍA: Se analizaron muestras de FCG de pacientes con periodontitis severa controlada, sometidos a un tratamiento de ortodoncia. Las muestras se tomaron: Antes del tratamiento de periodontitis, antes del tratamiento de ortodoncia, y entre 1 semana y 24 meses de haber iniciado el tratamiento. Luego, se cuantificaron los niveles de IL-6, IL-17A, MMP-8, RANKL y OPG presentes en el FGC mediante ELISA. RESULTADOS: Los niveles de IL-6, IL-17A, MMP-8, RANKL y OPG detectados no variaron entre los diferentes tiempos de evaluación del tratamiento de ortodoncia. Durante el tratamiento ortodóncico, la relación RANKL: OPG y los niveles de MMP-8 fueron menores en todos los tiempos analizados en comparación con los previos al tratamiento periodontal. Por el contrario, los niveles de OPG fueron más altos durante el tratamiento de ortodoncia que la periodontitis. CONCLUSIÓN: Los niveles de los biomarcadores no variaron durante el tratamiento de ortodoncia y estos fueron menores que los detectados durante la periodontitis. El tratamiento de ortodoncia no conllevo a cambios clínicos que afecten negativamente sobre el periodonto.BACKGROUND: Patients affected by periodontitis with pathologic migration of teeth need a treatment that controls periodontitis and returns teeth to an ideal position. The analysis of biomarkers related to the development of periodontitis can molecularly reflect the structural changes that occur teeth with reduced periodontium under orthodontic forces. The objective of the present study was to quantify the levels of IL-6, IL-17A, MMP-8, RANKL and OPG in the gingival crevicular fluid (GCF) of periodontitis-affected patients under orthodontic treatment. METHODS: GCF samples from periodontitis-affected patients under orthodontic treatment were analyzed. The samples were taken: Before periodontitis treatment, before orthodontic treatment, and between 1 week and 24 months after having started the orthodontic treatment. Then, the levels of IL-6, IL-17A, MMP-8, RANKL and OPG in the GCF were analyzed by ELISA. RESULTS: The detected levels of IL-6, IL-17A, MMP-8, RANKL and OPG did not significantly vary along the different evaluation time points. During orthodontic treatment, the RANKL:OPG relation and the MMP-8 levels were lower in all the time points, when compared with the ones analyzed before the periodontitis treatment. On the contrary, OPG levels were higher during the orthodontic treatment in comparison with before periodontitis treatment. CONCLUSION: The biomarkers’ levels did not vary during orthodontic treatment and these were lower than the levels detected during periodontitis. The orthodontic treatment did not lead to deleterious changes within the periodontium.
