Antecedentes: El área quirúrgica fue de las más afectadas por la pandemia COVID-19, en el Perú se adoptaron lineamientos como el reforzamiento de medidas de bioseguridad, y la suspensión de labores del personal vulnerable y cirugías electivas, lo cual sumado a las manifestaciones gastrointestinales de la COVID-19 pudo afectar la concordancia diagnóstica de las patologías de abdomen agudo. Objetivos: Describir la concordancia entre el diagnóstico preoperatorio y postoperatorio y las características epidemiológicas en patologías de abdomen agudo quirúrgico en pacientes COVID-19, durante el primer año de pandemia en el Hospital Nacional Arzobispo Loayza. Materiales y métodos: Se realizó un estudio de concordancia diagnóstica, transversal y retrospectivo. Se revisaron libros de reporte operatorio de pacientes mayores de 18 años con COVID-19 durante el periodo de abril de 2 020 a marzo de 2 021. Se procesó los datos en “STATA/SE 18.0” y se analizó la concordancia diagnóstica con el Índice Kappa y porcentaje de correlación, y además las variables de edad, sexo y frecuencias por patologías. Resultados: Se recolectaron los datos de 110 pacientes donde las patologías más frecuentes fueron Apendicitis aguda (61,82%), Colecistitis aguda calculosa (12,73%) y Obstrucción intestinal (7,27%). Se obtuvo una media de edad de 41,36 años y un predominio en varones con un 57,27%. Se obtuvo un k = 0,8879 y un porcentaje de correlación de 94,85%. Conclusiones: Se encontró una concordancia diagnóstica “casi perfecta” entre las tres patologías de abdomen agudo más frecuentes, determinando que el contexto de pandemia no afectó la precisión diagnóstica en emergencia. |

Background: The surgical area was one of the most affected by the COVID-19 pandemic. In Peru, guidelines were implemented, such as reinforcing biosecurity measures and suspending work activities for high-risk personnel and elective surgeries. This, combined with the gastrointestinal manifestations of COVID-19, could have affected the diagnostic concordance of acute abdomen pathologies. Objectives: Describe the concordance between preoperative and postoperative diagnoses and the epidemiological characteristics of acute abdomen pathologies in COVID-19 patients throughout the first year of the pandemic at Hospital Nacional Arzobispo Loayza in Lima, Peru. Materials and Methods: A retrospective and cross-sectional diagnostic concordance research was conducted. Operative reports of patients aged 18 and above with COVID-19 were reviewed for the period from April 2020 to March 2021. Data analysis was performed using STATA/SE 18.0, and diagnostic concordance was assessed using the Kappa coefficient and correlation percentage. Additionally, demographic variables such as age, sex, and pathology frequencies were examined. Results: Data from 110 patients were collected, with the most prevalent pathologies being Acute Appendicitis (61.82%), Acute Calculous Cholecystitis (12.73%), and Bowel Obstruction (7.27%). The mean age was 41.36 years, and there was a predominance of cases in men (57.27%). A Kappa coefficient of 0.8879 and a correlation percentage of 94.85% were obtained. Conclusions: The diagnostic concordance was deemed "Almost Perfect" for the three most frequent acute abdomen pathologies, suggesting that the pandemic context did not compromise diagnostic accuracy in emergency situations. |
