Introducción: La migraña infantil es una patología que se diagnostica clínicamente, en Perú no existen cuestionarios validados para su diagnóstico. Objetivos: adaptar culturalmente el cuestionario “The McMaster Pediatric Migraine Questionnaire”; realizar la traducción, retrotraducción, evaluación por expertos y test de comprensibilidad en el contexto nacional. Materiales y métodos: Estudio de adaptación cultural y validación de una escala en salud. Resultados: Concordancias entre traducciones: Kp = 0.4277 (p=0.0000); Kp = 0.4615 (p=0.0000), Kp = 0.6143 (p=0.0000), Kp = 0.3514 (p=0.0209), Kp= 0.3514 (p=0.0209) y Kp = 0.6322 (p=0.0002). Correlación entre traducciones: rho = 0.7289 (p=0.0000), rho= 0.7328 (p=0.0000), rho = 0.8165 (p=0.0000), r = 0.5536 (p=0.0010), r = 0.6352 (p=0.0001) y r = 0.8348 (p=0.0000). Concordancias entre retrotraducciones: Kp = 0.2911 (p=0.0015), Kp = 0.4062 (p=0.0001), Kp = 0.6461 (p=0.0000), Kp = 0.2066 (p=0.0274), Kp = 0.2453(p=0.0172), Kp = 0.7108; (p=0.0000). Correlación entre retrotraducciones: rho = 0.4812 (p=0.0000), rho = 0.5588; (p=0.0000), rho = 0.7143 (p=0.0000), rho = 0.5172 (p=0.0024), rho = 0.5320 (p=0.0017) y r = 0.6178 (p=0.0002). La evaluación por expertos de la validez de contenido fue 91.37% (IC al 95% de 88.1 a 94,6%). El test de comprensibilidad obtuvo una media de 86.3% (IC al 95% de 84.9 a 87,6%). Conclusiones: El cuestionario The McMaster Pediatric Migraine Questionnaire se adaptó culturalmente en Perú, la versión final fue revisada por expertos y demostró alta comprensibilidad por la población.
