Los estudios de supervivencia en hemodiálisis (HD) y diálisis peritoneal (DP) muestran diferencias y variabilidad de resultados. Objetivo: Determinar la supervivencia a mediano plazo de pacientes en HD y DP en un hospital del MINSA Callao, e identificar los factores que influyen. Métodos. Cohorte retrospectiva de pacientes > 18 años, incidentes con >3 meses en HD o DP (2012-2021). Variable principal supervivencia en meses, variable independiente tipo de terapia. Para comparar se utilizó chi2, y U de Mann Whitney. Se calculó Hazard Ratio (HR) según tipo de diálisis y covariables, regresión multivariada de Cox para evaluar riesgo de fallecer con HR ajustado (IC 95%). Se realizó estimaciones de Kaplan-Meier, curvas de supervivencia hasta 3 años, y Long Rank (p <0,05). Resultados. Hubo 901 pacientes, 749 HD (86,1%) y 152 DP (13,9%); más hombres en HD, más diabéticos e hipertensos en DP. Iniciaron con acceso temporal 69%. Fallecieron más en HD (47,3% vs 29,6%) p<0,005. La mediana de supervivencia fue 26 meses (RIC 12-51) HD y 33 meses (RIC 16-51,5) DP. La probabilidad acumulada de supervivencia a 3 años en HD fue 47% versus DP 64%. Hubo menor supervivencia (Kaplan Meier) y mayor probabilidad de fallecer (HR) en pacientes en HD, >60 años, diabéticos y con acceso temporal de diálisis (p<0.05). Conclusiones. Los pacientes con ERC5 que ingresaron a HD tuvieron menor supervivencia que los que ingresaron a DP. Hubo mayor riesgo de fallecer en pacientes diabéticos, mayores de 60 años y que iniciaron con acceso temporal de diálisis.Survival studies in hemodialysis (HD) and peritoneal dialysis (PD) show differences and variability in their results. The objective was to determine the medium-term survival of patients on HD and PD in a MINSA Callao hospital and to identify factors that influence. Methods. Retrospective cohort of incident patients > 18 years of age, with >3 months on HD or PD (2012- 2021). The main variable was survival in months, an independent variable was type of therapy. Chi2 and the Mann Whitney U test were used to compare. Hazard Ratio (HR) was calculated according to the type of dialysis and covariates, Cox multivariate regression to assess the risk of failure with HR adjusted (95% CI). Kaplan-Meier estimates, survival curves up to 3 years, and Long Rank test (p <0.05) were performed. Results. There were 901 patients, 749 HD (86.1%) and 152 PD (13.9%); more men on HD, more diabetics and hypertensives on PD. 69% started with a temporary catheter. There were more deaths on HD (47.3% vs 29.6%); p<0.005. Median survival was 26 months (IQR 12-51) on HD and 33 months (IQR 16-51.5) on PD. The cumulative probability of 3-year survival on HD was 47% versus PD 64%. There was lower survival (Kaplan Meier) and higher probability of dying (HR) in: patients on HD, >60 years, diabetics and with temporary dialysis access (p<0.05). Conclusions. Patients with CKD5 who were admitted to HD had lower survival than those who were admitted to PD. There was a higher risk of death in diabetic patients, over 60 years old and those who started with temporary dialysis access.
