El carcinoma urotelial de vejiga es una de las neoplasias más frecuentes del tracto urinario, caracterizada por su heterogeneidad clínica y patológica, lo que condiciona el pronóstico y la respuesta terapéutica. El objetivo del presente estudio fue describir las características clínico–anatomo–patológicas de los pacientes diagnosticados en el Instituto Nacional de Enfermedades Neoplásicas del Perú entre 2015 y 2019.Se realizó un estudio observacional, descriptivo, transversal y retrospectivo en 167 pacientes con diagnóstico histológico confirmado. La mediana de edad fue de 72 años, con predominio masculino (74.3 %). Entre los antecedentes, el 6.4 % presentaba diabetes mellitus, el 1.3 % consumo de alcohol y el 7.7 % hábito de fumar, porcentajes inferiores a los reportados en la literatura internacional. En cuanto a la ocupación, el 7.2 % eran agricultores y el 69.5 % no reportó actividad laboral. La mayoría procedía de Lima (18.6 %), Cajamarca (13.2 %) y Áncash (10.8 %).La sobrevida mínima fue de dos meses y la máxima de nueve años. El 24.6 % de los pacientes no superó el primer año tras el diagnóstico, mientras que el 37.1 % vivió más de tres años. El estadio clínico III fue el más frecuente (37.1 %), seguido del estadio I (22.7 %). El 41.3 % recibió tratamiento quirúrgico, principalmente cistectomía radical, y el 24 % quimioterapia adyuvante. En los casos operados, el patrón histológico más común fue el papilar (36.2 %), seguido del sólido o nodular (23.2 %). Además, se observó infiltración linfovascular en el 58 % y márgenes comprometidos en el 15.9 %.Estos resultados reflejan una cohorte predominantemente masculina, con diagnóstico en estadios avanzados y características histopatológicas asociadas a mal pronóstico.Urothelial carcinoma of the bladder is one of the most frequent neoplasms of the urinary tract and is characterized by marked clinical and pathological heterogeneity, which influences prognosis and therapeutic response. The objective of this study was to describe the clinical, anatomical, and pathological characteristics of patients diagnosed at the National Institute of Neoplastic Diseases (INEN) in Peru between 2015 and 2019.An observational, descriptive, cross-sectional, and retrospective study was conducted in 167 patients with histologically confirmed diagnoses. The median age at diagnosis was 72 years, with a predominance of males (74.3%). Among the recorded comorbidities, 6.4% had diabetes mellitus, 1.3% reported alcohol consumption, and 7.7% were smokers—percentages lower than those reported in international series. Regarding occupation, 7.2% were farmers, while 69.5% had no occupation reported. Most cases originated from Lima (18.6%), Cajamarca (13.2%), and Áncash (10.8%).The minimum survival was two months and the maximum nine years. A total of 24.6% of patients did not survive beyond the first year after diagnosis, whereas 37.1% lived for more than three years. Clinical stage III was the most frequent (37.1%), followed by stage I (22.7%). Surgical treatment was performed in 41.3% of patients, mainly through radical cystectomy, and adjuvant chemotherapy was administered in 24%. Among operated cases, the most common histological pattern was papillary (36.2%), followed by solid or nodular (23.2%). Lymphovascular invasion was found in 58% of cases, and positive margins in 15.9%.These findings describe a predominantly male cohort with a high proportion of advanced-stage diagnoses and pathological features associated with poor prognosis.
