Introducción: El tromboembolismo pulmonar es una afección común, especialmente en hospitalizados. La angiografía pulmonar por tomografía computarizada (CTPA) es la técnica diagnóstica preferida y se ha estimado como el gold standard, mientras que la SPECT-CT ha demostrado ser también una modalidad favorable siendo una opción que nos va a proporcionar información anatómica más detallada y precisa. Objetivo: Mapear las técnicas de la evidencia cientifica disponible sobre la precisión diagnóstica de la angiografía pulmonar por tomografía computarizada (CTPA) en comparación con la tomografía computarizada por emisión de fotón único (SPECT-CT) en el diagnóstico de tromboembolismo pulmonar en adultos y adultos mayores. A través de una revisión exploratoria de artículos científicos (Scoping Review). Metodología: La presente revisión de alcance (Scoping Review) se ejecutará teniendo en cuenta las pautas del Manual de Joana Briggs y la declaración PRISMA-Src para asegurar la calidad y rigurosidad del estudio. Se realizará una investigación exhaustiva de toda la información recopilada de PubMed, Mendeley, Scopus y Google Académico tomando en cuenta trabajos en inglés, portugués y español publicados entre el 2004 y 2024. Resultados: Se analizaron doce artículos de estudios retrospectivos, prospectivos y de revisión sistemática y metaanálisis. Los hallazgos muestran porcentajes de sensibilidad, especificidad, VPP y VPN, donde nos indican que presentan porcentajes similares con rangos de 85 % al 100% en algunos estudios y otros en los que hay diferencias significativas. Asimismo, se describe sus limitaciones y características técnicas de los estudios de CTPA y SPECT-CT, que permiten una interpretación adecuada y segura de la precisión diagnóstica. Conclusiones: La CTPA muestra mayor especificidad y valores predictivos positivos, mientras que SPECT-CT, especialmente en combinación con TC, ofrece sensibilidad comparable y mejor capacidad para descartar TEP. Ambos métodos son herramientas eficaces en el diagnóstico de TEP, aunque la elección puede depender de factores clínicos.Introduction: Pulmonary thromboembolism is a common condition, especially in hospitalized patients. Computed tomography pulmonary angiography (CTPA) is the preferred diagnostic technique and has been considered the gold standard, while SPECT-CT has also proven to be a favorable modality, being an option that will provide more detailed and accurate anatomical information. Objectivo: To map the available scientific evidence on the diagnostic accuracy of computed tomography pulmonary angiography (CTPA) compared to single-photon emission computed tomography (SPECT-CT) in the diagnosis of pulmonary thromboembolism in adults and older adults. Through a scoping review of scientific articles (Scoping Review). Methodology: The present Scoping Review will be carried out taking into account the guidelines of the Joana Briggs Manual and the PRISMA-Src statement to ensure the quality and rigor of the study. An exhaustive research of all the information collected from Pubmed, Mendeley, Scopus and Google Scholar will be carried out, taking into account papers in English, Portuguese and Spanish published between 2004 and 2024. Results: Twelve articles of retrospective, prospective and systematic review and meta-analysis studies were analyzed. The findings show percentages of sensitivity, specificity, PPV and NPV, which indicate that they present similar percentages with ranges from 85% to 100% in some studies and others in which there are significant differences. Likewise, it defines us their limitations and technical characteristics of CTPA and SPECT-CT studies, which allow an adequate and safe interpretation of diagnostic accuracy. Conclusions: CTPA shows higher specificity and positive predictive values, while SPECT-CT, especially in combination with CT, offers comparable sensitivity and better ability to rule out PTE. Both methods are effective tools in the diagnosis of PTE, although the choice may depend on clinical factors.
