Antecedentes: Las fracturas del tercio medio facial constituyen lesiones relevantes en cirugía maxilofacial por su complejidad anatómica, repercusión funcional y relación con diversos mecanismos traumáticos. Objetivo: Evaluar los factores asociados a las fracturas del tercio medio facial atendidas en el Servicio de Cirugía Bucal y Maxilofacial del Hospital Cayetano Heredia durante el periodo 2015–2023.Métodos: Estudio descriptivo, retrospectivo y transversal, realizado en 83 historias clínicas seleccionadas mediante muestreo no probabilístico por conveniencia. Se analizaron variables clínicas, temporales y sociodemográficas. Para el análisis descriptivo se calcularon frecuencias absolutas y porcentajes; asimismo, se aplicó la prueba de chi-cuadrado de Pearson para evaluar la asociación entre el tipo de fractura dicotómica y las variables estudiadas. Resultados: La categoría más frecuente fue otras fracturas aisladas y combinadas (60.3%), seguida de ZEB(33.7%). Las localizaciones anatómicas más comprometidas fueron cigoma/arco cigomático (63.9%), pared inferior/piso de órbita (57.8%) y pared lateral de órbita(56.6%). La causa más frecuente fue la violencia interpersonal (37.3%), seguida de accidentes de tránsito (26.5%) y caídas (24.1%). Predominaron los pacientes masculinos (73.5%), jóvenes de 15 a 29 años (33.7%), procedentes de Lima Metropolitana (59.0%), sin pareja (72.3%), ocupados (72.3%) y con instrucción secundaria (56.6%). Solo el lugar de procedencia mostró asociación significativa con el tipo de fractura dicotómica (p = 0.038). Conclusión: Las fracturas del tercio medio facial se presentaron principalmente como otras fracturas aisladas y combinadas, con compromiso frecuente de estructuras cigomáticas y orbitarias. El lugar de procedencia fue la única variable asociada significativamente al tipo de fractura dicotómica; sin embargo, este resultado debe interpretarse con cautela por la presencia de frecuencias esperadas bajas.

Background: Midface fractures are relevant injuries in maxillofacial surgery due to their anatomical complexity, functional impact, and association with different traumatic mechanisms. Objective: To evaluate the factors associated with midface fractures treated at the Oral and Maxillofacial Surgery Service of the Cayetano Heredia Hospital from 2015 to 2023. Methods: A descriptive, retrospective, and cross-sectional study was conducted using 83 medical records selected through non-probability convenience sampling. Clinical, temporal, and sociodemographic variables were analyzed. Absolute frequencies and percentages were calculated for the descriptive analysis, and Pearson’s chi-square test was used to evaluate the association between the dichotomous fracture type and the study variables. Results: The most frequent category was other isolated and combined fractures (60.3%),followed by ZEB fractures (33.7%). The most commonly affected anatomical siteswere the zygoma/zygomatic arch (63.9%), inferior orbital wall/floor (57.8%), andlateral orbital wall (56.6%). The most frequent cause was interpersonal violence(37.3%), followed by traffic accidents (26.5%) and falls (24.1%). Most patients were male (73.5%), aged 15 to 29 years (33.7%), from Metropolitan Lima (59.0%), without a partner (72.3%), employed (72.3%), and with secondary education (56.6%). Only place of origin showed a statistically significant association with the dichotomous fracture type (p = 0.038). Conclusion: Midface fractures occurred mainly as other isolated and combined fractures, with frequent involvement of zygomatic and orbital structures. Place of origin was the only variable significantly associated with the dichotomous fracture type; however, this finding should be interpreted with caution due to the presence of low expected frequencies.
