Objective: To determine the relationship between lower incisor inclination (IMPA)and the morphological characteristics of the mandibular symphysis (MS) usingcone beam computed tomography (CBCT) in patients with a Class I skeletalrelationship (SR) and different vertical facial patterns (VFP). Methodology: Across-sectional and retrospective study was conducted using 60 CT scans of ClassI patients, distributed as 20 for each VFP, classified according to the angles formedbetween the anterior cranial base (SN) and the Frankfort plane (Po-Or), both withrespect to the mandibular plane (Go-Me). Using Real Scan 2.0 software, angularand linear measurements were performed, including the IMPA and sixmeasurements for the MS: concavity (Id-B-Pg), convexity (B-Pg-Me), inclination(B-Pg-Md), height (Id-Me), prominence (Pg to B-Me), and vertical dimension (BB1-Gn). Results: Significant differences were found in the IMPA according to theVFP (p<0.001), with higher values in hypodivergent subjects (98.87° ± 8.63) andlower values in hyperdivergent subjects (87.06° ± 8.90). The MS of hyperdivergentindividuals exhibited greater convexity (138.20° ± 9.51 mm), height (31.38 ± 3.11mm), and inclination (59.02° ± 5.86 mm), while hypodivergent individuals showedgreater prominence (4.47 ± 0.95 mm) (p < 0.05). Concavity and vertical dimensiondid not show significant differences. The IMPA showed a weak negative correlationwith concavity (r = -0.29; p = 0.023) and height (r = -0.33; p = 0.010), and amoderate positive correlation with MS inclination (r = 0.55; p < 0.001).Conclusion: The morphology of the MS varies according to the VFP. The IMPAshowed a moderate positive correlation with the inclination of the scapula and weaknegative correlations with concavity and height. These findings highlight theimportance of jointly evaluating the IMPA and scapula morphology duringorthodontic and stability.
