Background: Recurrent endometrial cancer represents a therapeutic challenge due to the lack of standardized criteria for selecting candidates for cytoreductive surgery. Complete cytoreduction remains the primary surgical objective because of its association with improved oncologic outcomes. Objectives: To map the available evidence on the reported selection criteria for achieving complete cytoreduction in patients with recurrent endometrial cancer. Materials and Methods: A scoping review was conducted following the Joanna Briggs Institute methodology and the PRISMA-ScR guideline. A search was performed in MEDLINE, EMBASE, Scopus, the Cochrane Library, and grey literature. The search included studies published between 1995 and 2026 that evaluated patients with recurrent endometrial cancer who underwent cytoreductive surgery. Results: Thirteen retrospective studies involving 848 patients were included. Complete cytoreduction was achieved in more than 50% of cases across the evaluated cohorts. The most frequently reported criteria in patients with successful surgical outcomes were the presence of a solitary recurrence, isolated nodal involvement, smaller tumor size, younger age, and good performance status. Ascites, extensive peritoneal carcinomatosis, and high tumor burden were reported as unfavorable criteria for achieving complete cytoreduction. Conclusions: Selection criteria for cytoreductive surgery in recurrent endometrial cancer are multifactorial and involve clinical, surgical, and pathological variables.Limited tumor burden and localized patterns of recurrence were the most consistently reported criteria in patients who achieved complete cytoreduction.
