Criterios de selección para pacientes candidatas a citorreducción completa por cáncer de endometrio recurrente: una revisión exploratoria

Abstract

Introducción: El cáncer de endometrio recurrente representa un desafío terapéutico debido a la ausencia de criterios estandarizados para la selección de pacientes candidatas a cirugía citorreductora. La citorreducción completa constituye el principal objetivo quirúrgico por su asociación con mejores resultados oncológicos. Objetivos: Mapear la evidencia disponible sobre los criterios de selección reportados para el logro de una citorreducción completa en pacientes con cáncer de endometrio recurrente. Material y métodos: Se realizó una revisión exploratoria siguiendo la metodología del Joanna Briggs Institute y la guía PRISMA-ScR. Se efectuó una búsqueda en MEDLINE, EMBASE, Scopus, Cochrane Library y literatura gris. La búsqueda incluyó estudios publicados entre 1995 y 2026 que evaluaron pacientes con cáncer de endometrio recurrente sometidas a cirugía citorreductora. Resultados: Se incluyeron 13 estudios retrospectivos con 848 pacientes. La citorreducción completa fue alcanzada en más del 50% de los casos en las cohortes evaluadas. Los criterios más frecuentemente reportados en pacientes con éxito quirúrgico fueron la presencia de una recurrencia única, la afectación ganglionar aislada, un menor tamaño tumoral, una edad más joven y un adecuado estado funcional. La ascitis, la carcinomatosis peritoneal extensa y una elevada carga tumoral fueron reportados como criterios desfavorables para alcanzar una resección completa.

Background: Recurrent endometrial cancer represents a therapeutic challenge due to the lack of standardized criteria for selecting candidates for cytoreductive surgery. Complete cytoreduction remains the primary surgical objective because of its association with improved oncologic outcomes.Objectives: To map the available evidence on the reported selection criteria for achieving complete cytoreduction in patients with recurrent endometrial cancer. Materials and Methods: A scoping review was conducted following the Joanna Briggs Institute methodology and the PRISMA-ScR guideline. A search was performed in MEDLINE, EMBASE, Scopus, the Cochrane Library, and grey literature. The search included studies published between 1995 and 2026 that evaluated patients with recurrent endometrial cancer who underwent cytoreductive surgery. Results: Thirteen retrospective studies involving 848 patients were included. Complete cytoreduction was achieved in more than 50% of cases across the evaluated cohorts. The most frequently reported criteria in patients with successful surgical outcomes were the presence of a solitary recurrence, isolated nodal involvement, smaller tumor size, younger age, and good performance status. Ascites, extensive peritoneal carcinomatosis, and high tumor burden were reported as unfavorable criteria for achieving complete cytoreduction.Conclusions: Selection criteria for cytoreductive surgery in recurrent endometrial cancer are multifactorial and involve clinical, surgical, and pathological variables.Limited tumor burden and localized patterns of recurrence were the most consistently reported criteria in patients who achieved complete cytoreduction.
