Antecedentes: La necesidad de tratamiento odontológico en pacientes con enfermedades crónicas (EC) requieren conocimientos y actitudes renovadas del odontólogo para su correcta identificación. En Perú no existen estudios de referencia, que evalua las dimensiones para obtener la necesidad real, las competencias que necesita el odontólogo y otros factores que afectan la salud oral y general del adulto. Objetivo: Determinar la asociación entre enfermedades crónicas y necesidad de tratamiento odontológico en pacientes del Servicio de EPE del Centro Dental Docente (CDD) de la UPCH en Lima- Perú, 2016 - 2019. Materiales y métodos: Estudio transversal, retrospectivo, observacional y analítico. Se utilizó la base de datos del servicio de EPE (abril 2016 hasta diciembre 2019), la muestra fue de 1988 registros de adultos mayores de 18 años. Se evaluaron las variables: Necesidad de tratamiento odontológico, Enfermedades crónicas, tratamiento farmacológico, última visita al odontólogo, procedencia, grupo etáreo y sexo. La variable dependiente fue la necesidad de tratamiento odontológico la cuál fue validad por un juicio de expertos. Se realizó un análisis descriptivo, bivariado (Chi cuadrado) y multivariado (Regresión Log-lineal de Poisson). Resultados: Se encontró que la presencia de 3 a más enfermedades crónicas, consumo de más de 2 fármacos en adultos mayores tuvo asociación con la necesidad de tratamiento de Alta complejidad vs. Baja complejidad con un OR 4.54 (IC95 %: 2.03-10.17); Asimismo, cuando se ajustó el modelo se encontró la misma asociación ORa: 2.73 (IC95 %: 1.06-7.07). El 69.42% presentó de 2 a más EC, dos tercios de la población consumía de 2 a más fármacos, 56.14% fueron adultos mayores, el 65.39% de la población fueron de mujeres. Conclusión: Se evidencio que tener más de 3 enfermedades crónicas si impactará sobre una alta complejidad en el tratamiento odontológico versus un tratamiento de baja complejidad.Background: The need for dental treatment in patients with chronic diseases (CD) requires renewed knowledge and attitudes of the dentist for its correct identification. Objective: To determine the association between chronic diseases and the need for dental treatment in patients of the EPE Service of the Teaching Dental Center (CDD) of the UPCH in Lima- Peru, 2016 - 2019. Materials and methods: Cross-sectional, retrospective, observational and analytical study. The EPE service database was used (April 2016 to December 2019), the sample was 1988 records of adults over 18 years of age. The variables were evaluated: Need for dental treatment, Chronic diseases, pharmacological treatment, last visit to the dentist, origin, age group and sex. The dependent variable was the need for dental treatment, which was validated by expert judgment. A descriptive, bivariate (Chi square) and multivariate (Log-linear Poisson Regression) analysis was performed. Results: It was found that the presence of 3 or more chronic diseases, consumption of more than 2 drugs in older adults was associated with the need for treatment of High complexity vs. Low complexity with an OR 4.54 (95% CI: 2.03-10.17); Likewise, when the model was adjusted, the same ORa association was found: 2.73 (95% CI: 1.06-7.07). 69.42% presented 2 or more CDs, two thirds of the population consumed 2 or more drugs, 56.14% were older adults, 65.39% of the population were women. Conclusion: It was evidenced that having more than 3 chronic diseases would have an impact on a high complexity in dental treatment versus a low complexity treatment.
