Antecedentes: La muestra de orina, para algunos exámenes de laboratorio, es recolectado por el mismo paciente y es responsabilidad del personal de laboratorio garantizar la calidad del proceso preanalítico que asegure su utilidad para el diagnóstico y seguimiento de múltiples enfermedades. Objetivo: Evaluar la fase preanalítica en la recolección de orina de 24 horas en el laboratorio central de un hospital de Lima. Material y métodos: Estudio descriptivo y transversal que incluyo a los pacientes que acudieron al Hospital Cayetano Heredia. Con apoyo de los participantes se llenó una ficha de recolección de datos basada en la identificación de puntos críticos de la recolección de orina de 24 horas de Ruiz y col, y en la guía estandarizada de GP16-A3 y GP16A2 del CLSI. Se utilizó una hoja de Excel para realizar el análisis de frecuencias y los gráficos de barras. Resultado: Se obtuvieron 394 muestras de orina de 24 horas, 17 (4.3%) fueron rechazadas por el laboratorio por el principal motivo de inadecuada recolección de la muestra (94.1%). El error preanalítico más frecuente asociado al personal de salud fue la falta de anotación de la edad en la botella (90.98%), mientras que el error más frecuente del paciente fue la inadecuada rotulación de la hora de inicialización y finalización de la recolección (97.1%). Conclusión: Es necesario la estandarización de criterios de rechazo, y la mejora en la orientación de los pacientes para mejor adherencia a las recomendaciones del personal de laboratorio.Background: The urine sample, for some laboratory tests, is collected by the patient himself and it is the responsibility of the laboratory staff to guarantee the quality of the pre-analytical process that ensures its usefulness for the diagnosis and monitoring of multiple diseases. Objective: To evaluate the pre-analytical phase in the 24-hour urine collection in the central laboratory of a hospital in Lima. Material and methods: Descriptive and cross-sectional study that included patients who attended the Cayetano Heredia Hospital. With the support of the participants, a data collection form was filled out based on the identification of critical points in the 24-hour urine collection of Ruiz et al, and on the standardized guide GP16-A3 and GP16A2 of the CLSI. An Excel sheet was used to carry out the frequency analysis and the bar graphs. Result: 394 24-hour urine samples were obtained, 17 (4.3%) were rejected by the laboratory for the main reason of inadequate sample collection (94.1%). The most frequent pre-analytical error associated with health personnel was the lack of annotation of the age on the bottle (90.98%), while the most frequent error by the patient was the inadequate labeling of the start and end time of the collection (97.1 %). Conclusion: The standardization of rejection criteria is necessary, and the improvement in the orientation of patients for better adherence to the recommendations of laboratory personnel.
