Introducción: El dengue es un problema de salud pública endémico en Perú y otros países tropicales. A nivel individual, se sabe que el dengue severo está asociado a una segunda infección o a diferencias en la virulencia de los serotipos. Sin embargo, los factores que aumentan el riesgo de dengue severo a nivel poblacional aún son desconocidos. Por ello, en el primer estudio se utilizó data secundaria para estimar la población en riesgo de segundas infecciones, para luego explorar su asociación con la incidencia de dengue severo o dengue con señales alarma (DS-DCSA). También se exploró la asociación de la incidencia de DS-DCSA con otros predictores como la circulación de serotipos, el total de establecimientos de primer nivel de atención y las condiciones socioeconómicas de la región. Además, al no existir una vacuna efectiva contra el dengue para la población general, la principal estrategia para la prevención son las actividades de control vectorial del Ae. Aegypti en los hogares (fumigación, tratamiento focal y recojo de criaderos). Sin embargo, se ha observado renuencia a estas actividades. En el segundo estudio, se exploró la asociación entre la renuencia a cada una de las actividades de control vectorial y la experiencia cercana con DS-DCSA, así como su relación con otros potenciales predictores. Pregunta de la investigación 1: ¿Un mayor número estimado de personas en riesgo de una segunda infección está asociada a un mayor riesgo en la incidencia acumulada de DS-DCSA? Métodos: Es un estudio ecológico, observacional, analítico, longitudinal y retrospectivo. Se realizó análisis de bases de datos secundaria de casos de dengue e identificación de tipos de serotipo en muestras analizadas por el Ministerio de Salud. Se utilizó un modelo de regresión binomial negativa mixta para medidas repetidas en el tiempo para estimar la asociación entre la incidencia acumulada de DS-DCSA por 100 mil habitantes y la población estimada en riesgo de segundas infecciones, así como otros factores potencialmente asociados con el desenlace. Se analizaron los datos de 13 años (2007-2019) para 19 regiones del Perú. Se estimaron razones de incidencia (IRR) crudas y ajustadas utilizando el comando “menbreg” en Stata. Para la estimación de la asociación de interés, se consideraron variables de ajuste como el índice de pobreza, total de establecimientos de primer nivel de atención y tipos de serotipo circulantes (o combinaciones de serotipos). Resultados: En total se analizaron 13 años de datos desde el 2007 al 2019 en 19 regiones del Perú, haciendo un total de 247 años región. Se analizaron los datos de cerca de 40 mil muestras analizadas de identificación de tipo de serotipos, de los cuales el 58.5% de datos correspondían al serotipo DENV-2, el 27.0% al DENV-1, el 10.2% al DENV-3 y solamente el 4.3% al DENV-4. Con respecto a los casos de dengue analizados, el 86.4% correspondía a dengue sin señales de alarma, el 13.2% a dengue con señales de alarma y el 0.5% a dengue severo. Las 4 regiones con mayores incidencias de DS-DCSA y con mayor identificación de tipos de serotipos fueron Loreto, Piura, Madre de Dios y Ucayali. No se encontró una asociación significativa entre la población estimada en riesgo de segundas infecciones y la incidencia acumulada de DS-DCSA por 100 mil habitantes (IRRa 1.00, IC 1.00-1.01). Sin embargo, la circulación múltiple donde estuvo presente el serotipo DENV-2, se asoció a un mayor riesgo de incidencia acumulada de DS-DCSA por 100 mil habitantes (DENV-123: IRRa 6.62 IC 2.19 – 20.01; DENV-23: IRRa 3.87 IC 1.26 – 11.85). Pregunta de la investigación 2: ¿Existe asociación entre la experiencia cercana con DS-DCSA y la renuencia al control vectorial en hogares? Métodos: Es un estudio transversal vía encuesta a residentes con muestreo probabilístico en distritos endémicos de dengue de Piura (La Unión, Chulucanas y La Matanza) y Loreto (Iquitos y Punchana). Se estimaron razones de prevalencia (RP) crudas y ajustados utilizando el modelo de regresión logística ordinal entre la experiencia cercana con DS-DCSA y los niveles de renuencia (no renuente, renuente a veces y renuente siempre) a cada una de las actividades de control vectorial en hogares utilizando variables confusoras predefinidas. Resultados: Se aplicaron 883 encuestas. Solamente el 15% de la población de estudio tuvo experiencia cercana con DS-DCSA y se reportaron mayores niveles de renuencia (renuente siempre y renuente a veces) a la actividad del recojo de criaderos (54%), seguido de la fumigación (20%); el tratamiento focal tuvo menor nivel de renuencia (7%). En el modelo de regresión ajustada no se encontró la asociación entre la experiencia cercana con DS-DCSA y cada una de las actividades del control vectorial, sin embargo, la variable que mejor explicó los niveles de renuencia fue la percepción de la efectividad del producto o intervención. Percibir que el producto utilizado en la fumigación/tratamiento focal es efectivo se asoció a tener menos riesgo de ser renuentes a estas actividades (RPa 0.31 IC 0.17-0.0.56 para fumigación; RPa 0.23 IC 0.10-0.54 para tratamiento focal). Del mismo modo, tener una percepción de que el recojo de criaderos funciona se asoció a un menor riesgo de ser renuentes a esta actividad (RPa 0.32 IC 0.17-0.60). Conclusiones: La circulación del serotipo DENV-2 o su combinación se asociaron mayores razones de promedio de incidencia de DS-DCSA. Por otro lado, demostrar eficientemente la efectividad de las actividades de control vectorial puede disminuir la renuencia de la población.Introduction: Dengue is an endemic public health problem in Peru and other tropical countries. At the individual level, it is known that severe dengue is associated with a second infection or with differences in the virulence of the serotypes. However, the factors that increase the risk of severe dengue at the population level are still unknown. Therefore, in the first study, secondary data was used to estimate the population at risk of second infections and to later explore its association with the incidence of severe dengue or dengue with alarm signs (DS-DCSA). The association of the incidence of DS-DCSA with other predictors such as the circulation of serotypes, the total number of first-level care establishments, and the socioeconomic conditions of the region was also explored. In addition, since there is no effective vaccine against dengue for the general population, the main prevention strategy is vector control activities for Ae. Aegypti in homes (fumigation, focal treatment, and collection from breeding sites). However, hesitancy to these activities has been observed. In the second study, the association between hesitancy to each of the vector control activities and close experience with DS-DCSA was explored, as well as its relationship with other potential predictors. Research Question 1: Is a higher estimated number of people at risk of a second infection associated with a higher risk in the cumulative incidence of DS-DCSA? Methods: It is an ecological, observational, analytical, longitudinal, and retrospective study. Analysis of secondary databases of dengue cases and identification of serotype types in samples analyzed by the Ministry of Health was carried out. A mixed negative binomial regression model for repeated measures over time was used to estimate the association between the cumulative incidence of DS-DCSA per 100,000 inhabitants and the estimated population at risk of second infections, as well as other factors potentially associated with the outcome. Data from 13 years (2007-2019) for 19 regions of Peru were analyzed. Crude and adjusted incidence ratios (IRRs) were estimated using the “menbreg” command in Stata. To estimate the association of interest, adjustment variables such as the poverty index, the total number of primary care facilities, and circulating serotype types (or combinations of serotypes) were considered. Results: In total, 13 years of data were analyzed from 2007 to 2019 in 19 regions of Peru, making a total of 247 region years. The data of about 40 thousand samples analyzed for serotype identification were analyzed, of which 58.5% of the data corresponded to the DENV-2 serotype, 27.0% to DENV-1, 10.2% to DENV-3 and only 4.3% to DENV-4. Regarding the dengue cases analyzed, 86.4% corresponded to dengue without alarm signs, 13.2% to dengue with alarm signs, and 0.5% to severe dengue. The 4 regions with the highest incidence of DS-DCSA and with the highest identification of serotype types were Loreto, Piura, Madre de Dios, and Ucayali. No significant association was found between the estimated population at risk of second infections and the cumulative incidence of DS-DCSA per 100,000 inhabitants (IRRa 1.00, CI 1.00-1.01). However, multiple circulations where the DENV-2 serotype was present were associated with a higher risk of cumulative incidence of DS-DCSA per 100,000 inhabitants (DENV-123: IRRa 6.62 IC 2.19 – 20.01; DENV-23: IRRa 3.87 CI 1.26 – 11.85). Research question 2: Is there an association between close experience with DS-DCSA and hesitancy to vector control in households? Methods: It is a cross- sectional study via a survey of residents with probabilistic sampling in dengue-endemic districts of Piura (La Unión, Chulucanas, and La Matanza) and Loreto (Iquitos and Punchana). Crude and adjusted prevalence ratios (PR) were estimated using the ordinal logistic regression model between close experience with DS-DCSA and levels of hesitancy (not hesitancy, sometimes hesitancy, and always hesitancy) to each of the vector control activities in households using predefined confounding variables. Results: 883 surveys were applied. Only 15% of the study population had close experience with DS-DCSA and higher levels of hesitancy (always hesitancy and sometimes hesitancy) to the activity of collecting from breeding sites (54%), followed by fumigation (20% ); the focal treatment had a lower level of hesitancy (7%). In the adjusted regression model, no association was found between the close experience with DS-DCSA and each of the vector control activities; however, the variable that best explained the levels of hesitancy was the perception of the effectiveness of the product or intervention. Perceiving that the product used in fumigation/focal treatment is effective was associated with having less risk of being hesitancy to these activities (RPa 0.31 IC 0.17-0.0.56 for fumigation; RPa 0.23 IC 0.10-0.54 for focal treatment). In the same way, having a perception that the collection from hatcheries works was associated with a lower risk of being hesitancy to this activity (RPa 0.32 IC 0.17-0.60). Conclusions: The circulation of the DENV-2 serotype or its combination was associated with higher average incidence rates of DS-DCSA. On the other hand, efficiently demonstrating the effectiveness of vector control activities can decrease population hesitancy.
