Objetivo: Construir un instrumento breve (≤10 ítems) para el tamizaje de trastornos ansiosos y depresivos (TAD) en adultos del Perú. Método: Se realizó un análisis secundario de la base de datos del Estudio Epidemiológico de Salud Integral en Lima Metropolitana y el Callao–Replicación 2012 que incluyó numerosos ítems sobre diversa sintomatología psiquiátrica y aspectos de salud mental, la Escala de Distrés de Kessler-6 (K6), la percepción de necesidad de ayuda (PNA) y la identificación de TAD utilizando la Mini Entrevista Neuropsiquiátrica Internacional (MINI). Los participantes (N=4445) se dividieron aleatoriamente en 2 grupos. Con el primero (N=2181), se construyó un instrumento seleccionando los ítems principalmente en base a la combinación que generaba la mayor área bajo la curva ROC (abcROC) para la detección de TAD. Con el segundo (N=2264), se evaluó la validez del instrumento generado y de la K6 respecto a la presencia de TAD y de TAD con PNA considerando los valores de abcROC, sensibilidad (s), especificidad (e) e índice de Youden. Resultados: La escala resultante (EDTAD-9), de 9 ítems, tuvo un abcROC significativamente mayor que el de la K6 considerando como patrón de referencia a la presencia de TAD (0,8482 vs 0,8008) o la de TAD con PNA (0,9148 vs 0,8544). El punto de corte de la EDTAD-9 sugerido es >=6 para mujeres (s=75,34%, e=79,08%) y >=5 para hombres (s=80%, e=75,92%). Conclusiones: Las características psicométricas de la EDTAD-9 sugieren que puede ser útil para el tamizaje de TAD y especialmente de TAD con PNA.Objective: To construct a brief instrument (≤10 items) for the screening of anxiety and depressive disorders (ADD) in adults in Peru. Method: A secondary analysis of the database from the Epidemiological Study of Integral Health in Metropolitan Lima and Callao – Replication 2012 was conducted, which included numerous items on diverse psychiatric symptomatology and aspects of mental health, the Kessler-6 Distress Scale (K6), perceived need for help (PNH), and identification of ADD using the Mini International Neuropsychiatric Interview (MINI). The participants (N=4445) were randomly divided into 2 groups. With the first group (N=2181), an instrument was constructed by selecting items primarily based on the combination that generated the highest area under the receiver operating characteristic curve (AUC) for ADD detection. With the second group (N=2264), the validity of the generated instrument and the K6 was evaluated in relation to the presence of ADD and ADD with PNH considering the values of the AUC, sensitivity (se), specificity (sp), and Youden's index. Results: The resulting scale (SDADD-9), consisting of 9 items, had a significantly higher AUC than the K6 when considering as the reference standard the presence of ADD (0.8482 vs 0.8008) or ADD with PNH (0.9148 vs 0.8544). The suggested cut-off point for SDADD-9 is >=6 for women (se=75.34%, sp=79.08%) and >=5 for men (se=80%, sp=75.92%). Conclusions: The psychometric characteristics of the SDADD-9 suggest that it may be useful for the screening of ADD, particularly ADD with PNH.
