Develar por completo el mecanismo de acción de este antibiótico permitiría comprender los eventos que conducen a la resistencia; sin embargo, el entendimiento actual en este punto es limitado. Se han propuesto modelos que indican que la conversión de pirazinamida a ácido pirazinoico, el compuesto activo del antibiótico, por acción de la pirazinamidasa (gen pncA) es necesaria para su acción bactericida, aunque difieren en el mecanismo de acción posterior a esta conversión. Solo un modelo destaca la importancia del sistema de eflujo del compuesto activo, cuyo reingreso permite una acumulación intracelular que ocasiona la disrupción del potencial de membrana y la muerte bacteriana. Aquí, exploramos el rol de las bombas de eflujo Rv0191 y Rv1258c en el mecanismo de acción de la pirazinamida, y su potencial participación en una vía alternativa de resistencia, en ausencia de mutaciones en el gen pncA. Mediante el estado de represión y deleción de estos genes en Mycobacterium tuberculosis H37Rv, evidenciamos una reducción significativa en la concentración acumulada de ácido pirazinoico y la velocidad inicial de eflujo en el medio extracelular. Además de este efecto, se observó un crecimiento en presencia de hasta 1600 μg/ml de pirazinamida en un entorno ácido. El mecanismo de acción de la PZA en relación con el efecto de las bombas de eflujo en el transporte de ácido pirazinoico sugiere que el sistema de eflujo es multifactorial y que su alteración en la expresión génica puede conducir a un fenotipo resistente.The fight against tuberculosis faces challenges due to its lengthy treatment regimen and the presence of resistance to multiple antibiotics, including pyrazinamide, which can reduce the regimen from 12-9 months to 6 months. Fully elucidating the mechanism of action of this antibiotic would allow us to understand the events leading to resistance; however, our current understanding in this area is limited. Models have been proposed indicating that the conversion of pyrazinamide to pyrazinoic acid, the active compound of the antibiotic, by the action of pyrazinamidase (pncA gene) is necessary for its bactericidal action, although they differ in the mechanism of action following this conversion. Only one model highlights the importance of the efflux system of the active compound, whose reentry allows intracellular accumulation, causing disruption of membrane potential and bacterial death. Here, we explore the role of the Rv0191 and Rv1258c efflux pumps in the mechanism of action of pyrazinamide and their potential involvement in an alternative resistance pathway in the absence of mutations in the pncA gene. By repressing and deleting these genes in Mycobacterium tuberculosis H37Rv, we observed a significant reduction in the accumulated concentration of pyrazinoic acid and the initial efflux rate in the extracellular medium. Besides this effect, growth was observed in the presence of up to 1600 μg/ml of pyrazinamide in an acidic environment. The mechanism of action of PZA in relation to the effect of efflux pumps on the transport of pyrazinoic acid suggests that the efflux system is multifactorial and that its alteration in gene expression can lead to a resistant phenotype.
