En el tratamiento de ortodoncia se analizan ciertas características para obtener un correcto diagnóstico y justificar la elección de realizar un tratamiento con o sin extracciones. Objetivo: Evaluar qué características cefalométricas, dentoalveolares y de tejidos blandos están asociadas a la indicación de extraer piezas dentarias por motivos ortodónticos en el Centro Dental Docente Cayetano Heredia (CDDCH) durante los años 2004 al 2019.Materiales y métodos: La muestra fue de 1556 historias clínicas de pacientes atendidos en la CDDCH durante los años 2004 al 2019. Se recopilaron diversos datos, como medidas cefalométricas, dentoalveolares, tejidos blandos, sexo y edad de inicio del diagnóstico. Se empleó análisis de regresión logística para determinar asociación entre estas variables y la necesidad de extracciones de las cuatro primeras premolares (4-1PM) y extracciones totales. El modelo crudo se ajustó posteriormente, considerando únicamente aquellas características con una asociación significativa. Resultados: Se observa un OR significativo de extracciones totales en I.pp vestibularizados (OR 1.5), apiñamiento dentario inferior (OR 0.79), línea media superior e inferior (OR 1.2, OR 1.1), maloclusión clase II (OR 1.6), protrusión labial sup (OR 1.5), edad (OR 0.97) y de 4-1PM en I.pp vestibularizados (OR 1.4), IMPA vestibularizados (OR 1.38), OB (OR 0.9), apiñamiento dentario inferior (OR 0.8),maloclusión clase II y III (OR 0.5, OR 0.3), protrusión labial inferior (OR 2.2), edad (OR0.9). Conclusiones: La indicación de extracciones dentales se asociaron con la maloclusión clase II, III, protrusión labial superior e inferior, inclinación de los incisivos, OB, apiñamiento dentario inferior, edad. Se observó una disminución en la prevalencia de extracciones totales y de 4-1PM.In orthodontic treatment, certain characteristics are analyzed to obtain a correct diagnosis and justify the choice of performing treatment with or without extractions. Objective: To evaluate which cephalometric, dentoalveolar and soft tissue characteristics are associated with the indication to extract teeth for orthodontic reasons at the Cayetano HerediaTeaching Dental Center (CDDCH) during the years 2004 to 2019. Materials and methods: The sample was 1556 clinical records of patients treated at the CDDCH during the years 2004 to 2019. Various data were collected, such as cephalometric, dentoalveolar, soft tissue measurements, sex, and age at diagnosis. Logistic regression analysis was used to determine the association between these variables and the need for extractions of the first four premolars (4-1PM) and total extractions. The crude model was subsequently adjusted, considering only those characteristics with a significant association. Results: Asignificant OR of total extractions is observed in vestibularized I.pp (OR 1.5), lower dental crowding (OR 0.79), upper and lower midline (OR 1.2, OR 1.1), class II malocclusion (OR 1.6), protrusion sup labial (OR 1.5), age (OR 0.97) and 4-1PM investibularized I.pp (OR 1.4), vestibularized IMPA (OR 1.38), OB (OR 0.9), lower dental crowding (OR 0.8), class malocclusion II and III (OR 0.5, OR 0.3), lower lip protrusion (OR 2.2), age (OR 0.9). Conclusions: The indication for dental extractions was associated with class II, III malocclusion, upper and lower lip protrusion, inclination of the incisors, OB, lower dental crowding, and age. A decrease in the prevalence of total extractions and 4-1PM was observed.
