INTRODUCCIÓN: La Nefropatía Membranosa Idiopática (NMI) es una enfermedad autoinmune que puede ocasionar Síndrome Nefrótico por la formación de inmunocomplejos, se ha identificado el Receptor Fosfolipasa A2 tipo M (PLA2R) como autoantígeno involucrado, cuya ubicación es la membrana de los podocitos. El hallazgo de este autoantígeno en tejido renal y los anticuerpos séricos por ELISA han permitido la posibilidad contar con pruebas para el diagnóstico de la NMI; por otro lado, su utilidad para la evaluación de nuevas dianas terapéuticas como los anti CD20 (rituximab), han sido motivo de estudio para determinar valor diagnóstico y pronóstico. OBJETIVO GENERAL: Evaluar la evidencia científica disponible sobre el rendimiento diagnóstico y la utilidad clínica de las pruebas para la detección del PLA2R en pacientes con NMI. MÉTODO: Se realizó una revisión de Scoping Review utilizando la metodología PRISMA. RESULTADOS Y CONCLUSIONES: La detección del PLA2R (receptor fosfolipasa A2) por Inmunohistoquímica (IHC) o por inmunofluorescencia (IFA) en biopsias renales de los pacientes con Nefropatía Membranosa Idiopática (NMI) tiene rendimiento adecuado para su diagnóstico. La metodología del enzimoinmunoensayo ligado a enzimas (ELISA) para detectar anticuerpos anti PLA2R sérico en pacientes NMI y progresión a enfermedad renal crónica (ERC) es útil, pero su ausencia no lo descarta. La presencia del anti PLA2R no es un buen predictor para remisión parcial o completa, ya que es más útil para diagnóstico y el seguimiento en pacientes en tratamiento con rituximab.INTRODUCTION: Idiophatic Membranous nephropathy (IMN) is an autoimmune disease that can cause nephrotic syndrome due to the formation of immune complexes. The phospholipase A2 receptor type M (PLA2R) has been identified as the autoantigen involved, located in the podocyte membrane. The discovery of this autoantigen in renal tissue and serum antibodies by ELISA have made it possible to have tests for the diagnosis of MN; on the other hand, its usefulness for the evaluation of new therapeutic targets such as anti CD20, (rituximab), have been the subject of study to determine their diagnostic and prognostic value. GENERAL OBJECTIVE: To evaluate the available scientific evidence regarding the diagnostic performance and clinical utility of tests for the detection of the phospholipase A2 receptor (PLA2R) in patients with idiopathic membranous nephropathy (IMN). METHOD: A scoping review was conducted using the PRISMA methodology. RESULTS AND CONCLUSIONS: The detection of PLA2R detected by immunohistochemistry (IHC) or immunofluorescence in renal biopsies of patients with primary Idiopathic Membranous Nephropathy (IMN) shows adequate diagnostic performance. The enzyme-linked immunoassay (ELISA) methodology for detecting serum anti-PLA2R antibodies in NMI patients and progression to chronic kidney disease (CKD) is useful, but its absence does not rule it out. The presence of anti-PLA2R antibodies is not a good predictor for partial or complete remission, being more useful for diagnosis than for monitoring response to rituximab treatment.
